The D-dimer test is a blood test that measures fragments released when a blood clot breaks down. The D-dimer test is used mainly to help rule out deep vein thrombosis (DVT) or pulmonary embolism (PE) in people assessed as low or intermediate risk. A raised D-dimer is non-specific and always needs imaging or further assessment.
ቁልፍ ማውጫዎች
- The D-dimer test is a rule-out test, not a rule-in test, it is most useful when the result is negative and clinical suspicion is already low.
- A raised D-dimer does not confirm a clot. Infection, surgery, trauma, pregnancy, cancer, liver disease and older age all raise D-dimer.
- A negative D-dimer does not exclude a clot when clinical probability is high; imaging is needed regardless of the D-dimer result in such cases.
- D-dimer cut-offs, units and assay methods differ between laboratories, so results from different labs are not directly comparable.
- Blood collection takes a few minutes; the D-dimer test itself does not tell you where a clot is.

- D dimer blood test
- Fibrin degradation fragment test
- Quantitative D-dimer / high-sensitivity D-dimer assay
- Commonly asked for in India as "clot test", "DD test" or "D-dimer report" alongside coagulation profile (PT/INR, aPTT)
What the D-dimer test is
When the body forms a clot, it lays down a mesh of cross-linked fibrin. When that clot is broken down again by the enzyme plasmin, small fragments are released into the blood. One of those fragments is D-dimer. The D-dimer test measures the concentration of these fragments.
D-dimer therefore reflects clot formation ና breakdown happening somewhere in the body. It does not say where, why, how big or how old the clot is. It cannot distinguish a clot in a leg vein from fibrin turnover caused by pneumonia, a recent operation or a bruise.
Different laboratories use different assay platforms. Some report in D-dimer units and some in fibrinogen equivalent units, which are roughly double. Because of this, a number from one laboratory should not be compared directly with a number from another. Always read the result against the reference range printed on that specific report.
Why the D-dimer test is done
- የተጠረጠረ ጥልቅ የደም ሥር ቲምብሮሲስ (DVT): painful, swollen, warm calf or thigh. A negative high-sensitivity D-dimer in a patient scored as unlikely for DVT makes DVT sufficiently improbable that leg ultrasound may be avoided.
- Suspected pulmonary embolism (PE): sudden breathlessness, pleuritic chest pain, unexplained fast heart rate. D-dimer is used after clinical probability scoring (such as Wells or Geneva), not before it.
- Suspected disseminated intravascular coagulation (DIC): D-dimer forms one component of a DIC scoring panel alongside platelet count, prothrombin time and fibrinogen. D-dimer alone does not diagnose DIC.
- Deciding on duration of anticoagulation: in some patients with unprovoked venous thrombosis, a D-dimer measured after stopping anticoagulation can help a specialist judge recurrence risk. This is a specialist decision, not a routine one.
- Suspected aortic dissection: D-dimer is sometimes used as part of risk assessment, but it does not replace urgent imaging when dissection is suspected.
ይህንን ፈተና ማን መውሰድ አለበት
- People with symptoms suggesting DVT or PE whose doctor has assessed them as low or intermediate clinical probability.
- Patients being evaluated for DIC in the context of severe sepsis, major trauma, obstetric complications or certain cancers.
- Selected patients with unprovoked venous thromboembolism being assessed by a haematologist for how long to continue anticoagulation.
Who should generally not get this test
- ሰዎች በ ከፍ ያለ clinical probability of PE or DVT, they need imaging directly, because a negative D-dimer will not change management.
- Asymptomatic people seeking a "clot screening" test. D-dimer is not a screening test in healthy people and frequently produces raised results that lead to unnecessary scans.
- People in whom a raised result is near-inevitable and uninterpretable, for example immediately after major surgery, unless a specific clinical question is being asked.
What the D-dimer test cannot detect or exclude
- It cannot locate a clot. A raised D-dimer gives no information about which vessel is involved. Imaging is required.
- It cannot confirm DVT or PE. Specificity is low. Many people with raised D-dimer have no clot at all.
- It cannot reliably exclude clot in high-probability patients. In this group a normal D-dimer still leaves an unacceptable residual risk, so imaging proceeds anyway.
- Small, distal or older clots may not raise D-dimer enough. Small isolated calf vein thrombi, small subsegmental pulmonary emboli and clots that are several days or weeks old can be associated with a normal or near-normal result.
- Anticoagulation lowers D-dimer. A patient already on heparin, warfarin or a direct oral anticoagulant may have a normal D-dimer despite thrombosis.
- It does not assess bleeding risk and is not a substitute for PT/INR, aPTT or platelet count.
- It does not diagnose cancer, COVID-19, heart attack or stroke, even though D-dimer can rise in all of these.
A "not detected" or normal D-dimer describes what the assay measured in that sample at that moment. It is not the same as a clot having been excluded. If your breathlessness, chest pain or leg swelling continues or worsens despite a normal D-dimer, go back to your doctor.
Related but different examinations: how the D-dimer test compares
These tests answer different questions. None of them is universally superior; the right first test depends on the clinical presentation and how likely a clot is judged to be.
| ሙከራ | የሚል ጥያቄ ይመልሳል | Key difference from D-dimer |
|---|---|---|
| ዲ-ዲመር | Is clot breakdown occurring anywhere in the body? | Sensitive but non-specific; useful mainly to exclude clot in lower-probability patients |
| Doppler ultrasound of the leg (venous duplex) | Is there a clot in this specific leg vein? | Directly images the vein; less reliable for pelvic veins and isolated calf veins |
| ሲቲ pulmonary angiography (CTPA) | Is there a clot in the pulmonary arteries? | Confirms or excludes PE; involves radiation and iodinated contrast, with caution in kidney impairment and pregnancy |
| የአየር ማናፈሻ-ፐርፊሽን (V/Q) ቅኝት | Is lung perfusion mismatched, suggesting PE? | Alternative to CTPA in pregnancy or contrast allergy; less conclusive when the chest X-ray is abnormal |
| PT/INR and aPTT | How well does blood clot? Is bleeding risk raised? | Measures clotting capacity, not clot breakdown, the opposite question |
| Fibrinogen | How much clotting protein is available? | Falls in DIC while D-dimer rises; used together, not interchangeably |
| FDP (fibrin degradation products) | Is fibrin or fibrinogen being broken down? | Less specific than D-dimer; D-dimer reflects breakdown of cross-linked fibrin only |
| ትሮፖኒን | Is there heart muscle injury? | Different organ, different question; sometimes ordered alongside D-dimer in chest pain |
Also distinguish the qualitative bedside/point-of-care D-dimer from the quantitative high-sensitivity laboratory assay. Only high-sensitivity quantitative assays are validated for safely excluding PE within diagnostic pathways.
How to prepare for the D-dimer test
Preparation is minimal, but instructions are set locally by each laboratory. Where your collection centre's instructions differ from anything below, follow your collection centre.
- ጾም፡- generally not required for the D-dimer test. Eat and drink normally unless the same blood draw includes tests that do need fasting.
- መድሃኒቶች: tell the requesting doctor and the phlebotomist about anticoagulants (heparin, warfarin, apixaban, rivaroxaban, dabigatran), antiplatelets and any thrombolytic treatment. Do not stop or change any prescribed medicine for this test, speak to the prescriber first.
- የቅርብ ጊዜ ክስተቶች፡ mention recent surgery, trauma, fracture, hospital admission, infection, long-distance travel, active cancer treatment or a recent pregnancy loss or delivery, as all of these influence interpretation.
- Hydration and veins: being reasonably hydrated makes the draw easier. If you have a fluid restriction for heart failure or kidney disease, do not increase your fluid intake for this test, keep to the limit your doctor set.
- አልባሳት: a loose sleeve that rolls above the elbow helps.
እርግዝና
D-dimer rises progressively through normal pregnancy and remains raised in the early postpartum period. Standard cut-offs perform poorly in pregnancy, and pathways for suspected PE in pregnancy differ. Tell the team you are pregnant or recently delivered before the sample is taken.
ልጆች
D-dimer is used far less often in children and reference intervals differ by age, particularly in newborns. Paediatric results need paediatric interpretation.
የቆዩ አዋቂዎች
D-dimer rises with age. Many clinicians use an age-adjusted cut-off for patients above 50 years when assessing suspected PE, which reduces unnecessary imaging. Whether an age-adjusted threshold is applied depends on the assay and the local protocol.
People already on anticoagulants
Anticoagulation reduces D-dimer and can produce a falsely reassuring result. The test is much less informative in someone already on treatment. Continue your prescribed medicine and let the doctor know you are taking it.
የኩላሊት ወይም የጉበት በሽታ
D-dimer tends to be higher in advanced chronic kidney disease and in chronic liver disease, independent of any clot. This lowers the usefulness of a standard cut-off in these groups.
What happens during the D-dimer test
- ምዝገባ እና ቼክ፡ your details and the requesting doctor's instructions are confirmed.
- የደም ስብስብ; a tourniquet is applied, the skin at the inner elbow is cleaned, and blood is drawn into a citrate tube, usually light blue topped. The tube must be filled to the marked line, as under-filling can invalidate the sample.
- የሚፈጀው ጊዜ: the needle is usually in the vein for under a minute. The whole draw takes about five minutes, and most people are in and out of the collection area within 15-30 minutes.
- ከዚያ በኋላ፦ pressure is applied for a minute or two and a small dressing is placed. Normal activity can resume immediately. Avoid heavy lifting with that arm for an hour or so if the site is sore.
- በመስራት ላይ: the sample is centrifuged and analysed on an automated coagulation analyser. In emergency departments, D-dimer is commonly run urgently because the result affects immediate decisions; routine outpatient turnaround is longer and is set by each laboratory.
Understanding your D-dimer report
Read the number against the reference range and the unit printed on your own report. Two things must match before any comparison makes sense: the unit system (DDU or FEU) and the assay platform.
- Result below the laboratory cut-off: in a patient whose doctor has already judged clot to be unlikely, this makes DVT or PE improbable enough that imaging is often not needed. It does not mean no clot can possibly exist.
- Result above the laboratory cut-off: this means fibrin is being broken down somewhere. It is common, frequently has a non-clot explanation, and by itself is not a diagnosis. The usual next step is targeted imaging based on symptoms, leg Doppler for suspected DVT, CTPA or V/Q scan for suspected PE.
- Very high results: higher numbers are statistically more likely to be associated with thrombosis, but no single number confirms a clot, and there is no threshold above which imaging can be skipped.
The D-dimer test is never interpreted in isolation. It is interpreted alongside your symptoms, examination findings, risk factors and clinical probability score by the doctor who requested it. Results vary depending on individual clinical circumstances.
When D-dimer results can be misleading
Causes of a falsely raised or non-specifically raised D-dimer
- Increasing age, even without disease
- እርግዝና እና የድህረ ወሊድ ጊዜ
- Recent surgery, trauma, fracture or significant bruising
- Infection and sepsis, including severe pneumonia and dengue
- አክቲቭ ካንሰር
- Chronic liver disease and advanced chronic kidney disease
- Recent heart attack, stroke, atrial fibrillation or heart failure
- Inflammatory and autoimmune disease
- Extensive haematoma, burns or recent thrombolysis
- Rheumatoid factor and certain heterophile antibodies interfering with the assay
- Markedly haemolysed or lipaemic samples, depending on the platform
Causes of a falsely lowered D-dimer
- Anticoagulant treatment already started
- Clot that is small, isolated to distal calf veins, or subsegmental
- Symptoms present for many days, by which time D-dimer may have fallen
- Under-filled citrate tube diluting the sample
- Delayed transport or incorrect storage of the sample
- Very high lipid or bilirubin levels interfering with some optical assays
- Rarely, the "hook effect" at extremely high concentrations on certain assays
Risks and safety of the D-dimer test
The D-dimer test needs only a standard venous blood sample. The risks are those of any venepuncture and are usually minor.
- የተለመዱ እና ጥቃቅን: brief stinging, a small bruise, mild soreness at the puncture site.
- ብዙም ያልተለመደ feeling faint or lightheaded, a larger bruise (more likely if you take anticoagulants or antiplatelets), a second attempt being needed if the vein is difficult.
- ያልተለመደ፡ a small collection of blood under the skin, local infection, or temporary nerve irritation causing tingling.
Tell the phlebotomist beforehand if you have fainted during blood tests previously, have a bleeding disorder, take blood thinners, or have lymphoedema or a dialysis fistula in one arm so the other arm is used.

ልዩ ሁኔታዎች
እርግዝና እና የድህረ ወሊድ ጊዜ
D-dimer rises normally across the trimesters, so a raised result in pregnancy often reflects pregnancy itself. Suspected PE in pregnancy is assessed using pathways designed for pregnancy, which may involve leg Doppler first, then a V/Q scan or CTPA with dose considerations. Decisions here are made by the obstetric and medical teams together.
ልጆች
Venous thrombosis is uncommon in children and usually occurs with a central line, serious infection or an underlying condition. D-dimer has not been validated in children the way it has in adults, so paediatric decisions rely much more on imaging and clinical assessment.
የቆዩ አዋቂዎች
Because the baseline D-dimer rises with age, a fixed cut-off labels many healthy older people abnormal. An age-adjusted threshold is often used for suspected PE above 50 years to reduce unnecessary scanning, when the assay supports it.
የስኳር በሽታ ያለባቸው ሰዎች
No change to glucose-lowering medicines is needed for the D-dimer test, since fasting is not usually required. If the blood draw is combined with fasting tests, discuss the timing of insulin or sulphonylureas with the prescriber first to avoid hypoglycaemia.
የሰደደ የኩላሊት በሽታ
D-dimer is often higher in advanced CKD without any clot. If imaging is then needed, contrast use for CTPA requires balancing against kidney function, which the treating team will assess.
የልብ ችግር
D-dimer can be raised in decompensated heart failure and atrial fibrillation, and breathlessness from heart failure can mimic PE. Do not increase fluid intake before the test if you are on a fluid restriction.
ነቀርሳ
Active cancer both raises D-dimer and raises genuine clot risk, so the test loses much of its ability to discriminate. Imaging is often chosen earlier in people with cancer.
The D-dimer test in the Indian context
- Infection-driven elevations are common. Dengue, enteric fever, tuberculosis, severe pneumonia and sepsis all raise D-dimer. In a febrile patient, a raised D-dimer is frequently explained by the infection rather than by a clot.
- Post-COVID testing habits. D-dimer became widely ordered during the pandemic and is still sometimes requested in people with no clot symptoms. Outside an acute clinical question, a D-dimer in a well person often causes anxiety and unnecessary CT scans rather than clarity.
- Venous thromboembolism is under-recognised. DVT and PE were historically considered uncommon in South Asians, and that assumption still delays diagnosis. Post-surgical, post-partum and immobility-related thrombosis does occur and should be considered when symptoms fit.
- Rheumatic heart disease and atrial fibrillation remain relatively common and can be associated with raised D-dimer and with cardioembolic events.
- Anaemia and haemolysed samples. Difficult draws and sample haemolysis can affect some assay platforms; a repeat sample may be requested.
- Assay variation between labs. Indian laboratories use several different platforms and unit systems. Carry the original report rather than only the number when seeing a new doctor, and expect a repeat rather than a direct comparison.
በህንድ ውስጥ ወጪ እና ኢንሹራንስ
The D-dimer test is a commonly available laboratory test, priced in the low-to-middle range for coagulation investigations. Charges differ substantially between cities, between standalone collection centres and hospital laboratories, and between routine and urgent processing. Ask the laboratory for its current rate before the sample is taken.
በዋጋው ላይ ምን ተጽዕኖ ያሳድራል:
- Assay platform used, high-sensitivity quantitative assays generally cost more than qualitative ones
- Urgent or stat processing versus routine batching
- Whether the test is standalone or bundled into a coagulation or emergency panel
- Hospital versus standalone laboratory pricing, and city tier
- የቤት ናሙና መሰብሰብ ክፍያዎች
- የላብራቶሪ እውቅና ሁኔታ
ኢንሹራንስ when the D-dimer test is performed during an admission or day-care episode, it is usually covered as part of the hospitalisation claim, subject to policy terms. Outpatient investigations are often covered only if the policy includes an OPD benefit or a specific diagnostic cover. Pre-existing disease clauses and waiting periods may apply. Confirm with your insurer or the hospital's insurance desk before assuming cover.

ተደጋግሞ የሚነሱ ጥያቄዎች
የዲ-ዲመር ምርመራ ለምን ጥቅም ላይ ይውላል?
The D-dimer test helps doctors decide whether deep vein thrombosis or pulmonary embolism is unlikely enough to avoid imaging, in patients already assessed as low or intermediate risk. The D-dimer test also contributes to scoring systems for disseminated intravascular coagulation, alongside platelet count, prothrombin time and fibrinogen.
How long does the D-dimer test take?
The blood draw itself takes roughly five minutes, and most people spend 15-30 minutes at the collection centre including registration. Laboratory analysis is quick on automated analysers, and emergency departments usually process D-dimer urgently. Routine outpatient reporting time is set by each laboratory, ask when you give the sample.
Do I need to fast before a D-dimer test?
Fasting is not usually required for the D-dimer test, so you can eat and drink normally. If the same visit includes fasting tests such as lipids or fasting glucose, follow those instructions instead. If you take insulin or diabetes tablets and are asked to fast, discuss timing with your prescriber first.
My D-dimer is raised but my scan was normal. What now?
This is a common and usually reassuring combination. A raised D-dimer with normal targeted imaging most often reflects infection, inflammation, recent surgery, pregnancy, age or another non-clot cause. Your doctor will look for that explanation. Return promptly if breathlessness, chest pain or leg swelling develops or worsens.
Can the D-dimer test be wrong?
The D-dimer test is sensitive but not specific, so raised results frequently occur without a clot. Falsely low results can occur with anticoagulation, small or distal clots, symptoms lasting many days, or an under-filled sample tube. Interpretation always depends on clinical probability assessed by your doctor.
Is D-dimer the same as a coagulation profile or PT/INR?
No. PT/INR and aPTT measure how well blood forms a clot and help assess bleeding risk or warfarin dosing. The D-dimer test measures fragments left after a clot breaks down. They answer opposite questions and are not interchangeable, although both may be requested together.
Should I get a D-dimer test if I feel well but am worried about clots?
Generally no. The D-dimer test is not validated as a screening test in people without symptoms. In well people it often returns a mildly raised, unexplained value that leads to anxiety and avoidable CT scanning. Discuss your specific risk factors with a doctor instead.
Does being on blood thinners affect the D-dimer result?
Yes. Anticoagulants such as heparin, warfarin, apixaban, rivaroxaban and dabigatran reduce D-dimer levels and can produce a falsely reassuring result. Tell the laboratory and the requesting doctor what you take. Do not stop or adjust any anticoagulant for a test without speaking to the prescriber.
Why is my D-dimer high in pregnancy?
Pregnancy naturally increases clotting factor activity and fibrin turnover, so D-dimer rises through the trimesters and stays raised after delivery. Standard cut-offs therefore perform poorly in pregnancy. Suspected clot in pregnancy is assessed using pregnancy-specific pathways, often starting with leg Doppler ultrasound.
