    What do you need to know about TLH surgery?

    General anesthesia is given where the surgeon makes a small incision near the belly button. The size of the incision depends on the size of the uterus and the type of laparoscope used for inserting through the incisions made around the belly button area. The abdomen is inflated with carbon dioxide to visualize the abdomen and pelvis using the laparoscope.

    The use of this laparoscope will liberate the uterus from ligaments and surrounding blood vessels. To release the uterus and cervix, an incision will be created at the top of the vagina; the incision will then be closed with dissolvable sutures, and blood vessels will be cleansed to prevent bleeding. To remove a large uterus, the instrument called Morcellator is used to divide it into smaller sections and then taken out through the incisions.

    What are the symptoms that can lead to TLH?

    Here are the common symptoms that can lead to a surgeon suggesting total laparoscopic hysterectomy:

    • Fibroids – Pelvic pain, severe abdominal uterine hemorrhage, painful intercourse, and other symptoms that are caused by non-cancerous tumors.
    • Endometriosis – Pelvic discomfort is caused by the expansion of the uterine lining in other areas of the abdomen or the uterine muscle.
    • Uterine prolapse – The downward movement of the uterus into the vaginal canal

    What are the possible complications of the TLH procedure?

    Here is the list of certain specific complications that can arise from TLH procedure:

    • Organ injury – During the surgery, any organ in the abdomen or pelvis could be injured by accident.
    • Infections – Though antibiotics are given to the patient before the surgery through IV, certain infections can occur. Bladder infection, or UTI, is a common type of infection. that can cause a burning sensation when passing urine, the presence of blood in the urine, fever, chills and pelvic or back pain.
    • Vascular injury – Since instruments are inserted into the abdomen, there is a potential risk of injury during TLH.
    • Blood loss or transfusion – The blood loss involved in TLH procedure is moderate. Sometimes, the blood loss can be more and a blood transfusion will be needed. This causes a risk of Hepatitis or HIV.
    • Hematoma – A patch of stagnant blood is called hematoma and this happens when a blood vessel continues to bleed after the surgery is completed. Surgical drainage may be required if too much blood has been accumulated within a short amount of time, however, this is uncommon.
    • Chronic pain – A patient can acquire chronic pain in a region that has undergone surgery after any operation. The pain usually goes away with time, though some numbness may remain.

    What can you expect as a result of the TLH procedure?

    TLH takes about 1-2 hours on average, and some patients can return home the same day of surgery, while others spend one night in the hospital before being discharged the next day. Even though patients can be discharged within 1-2 days, TLH surgery will require two to four weeks of proper rest at home.

    Following TLH, some patients may experience pain reduction and the correction of abdominal uterine hemorrhage. A pathology report is necessary to review with your doctor and see if any more treatment is required. Patients who experienced uterine prolapse previous to surgery will need medication to get relief from vaginal pressure and pelvic pain.
