Neurosciences at Apollo Hospitals, Gurugram, brings together medical and surgical care for conditions affecting the brain, spinal cord, spine, nerves and muscles. Patients may seek care for stroke, seizures, headache, Parkinson's disease, memory problems, nerve or muscle weakness, brain and spine tumors, or injuries involving the head and spine.

Neurological symptoms can have several causes, and some require urgent treatment. A sudden change in speech, facial weakness, loss of movement, a prolonged seizure, severe head injury or new loss of bladder control with limb weakness needs emergency assessment. Neurologists, neurosurgeons, neurointerventionists, critical care specialists, radiologists and rehabilitation professionals may work together according to the diagnosis.

Care can range from clinical assessment and medicines to catheter-based treatment, surgery, critical care and rehabilitation. Imaging and other tests are selected for the individual rather than applied as a standard package. Treatment plans take into account the diagnosis, severity, other medical conditions and the patient's priorities.

Stroke and Cerebrovascular Conditions
A blood clot blocks an artery supplying part of the brain. Symptoms can include sudden facial drooping, weakness or numbness on one side, difficulty speaking, loss of balance or sudden visual loss. Call 112 and reach an emergency department immediately. Eligible patients may receive intravenous clot-dissolving treatment within a defined time window, while selected large-vessel strokes can be treated with mechanical thrombectomy based on imaging and clinical criteria.
Bleeding within or around the brain can cause sudden headache, weakness, vomiting, seizures or reduced consciousness. Causes include uncontrolled high blood pressure, aneurysm rupture and other blood-vessel abnormalities. Emergency imaging determines whether medical treatment, a catheter-based procedure or surgery is needed.
A TIA causes temporary stroke-like symptoms without evidence of acute infarction. Symptoms may resolve quickly, but emergency assessment is still needed because the risk of a subsequent stroke can be highest soon after the event.
  • Brain aneurysm and vascular malformation: An aneurysm is a weakened area of an artery, while an arteriovenous malformation is an abnormal connection between arteries and veins. They may be found incidentally or present with headache, seizures or bleeding. Treatment or monitoring depends on the anatomy and estimated risk.
  • Cerebral venous thrombosis: A clot in the veins that drain the brain can cause persistent headache, seizures, visual symptoms or neurological weakness. Risk factors include pregnancy, the period after childbirth, infection, inflammatory disease and clotting disorders, although some patients have no clear trigger.
Migraine can cause moderate or severe headache, nausea and sensitivity to light or sound. Some attacks include visual, sensory or speech symptoms known as aura. Treatment may include medicines for individual attacks, preventive therapy and management of contributing factors.
This commonly causes pressure or tightness on both sides of the head and is usually mild to moderate. Diagnosis is based on the pattern and examination rather than on a single scan.
  • الصداع العنقودي: Cluster headache causes severe pain around one eye in repeated bouts, often with tearing, nasal blockage or restlessness on the same side. Its acute and preventive treatments differ from those used for migraine.
Frequent use of medicines for headache can make headaches more persistent. Treatment involves reviewing the medicines, reducing the overused drug safely and starting preventive care when appropriate.
  • Headache red flags: Urgent assessment is needed for a sudden severe headache, headache with fever and neck stiffness, new weakness or confusion, seizure, visual loss, recent head injury, pregnancy-related concerns or a rapidly changing headache pattern.
Epilepsy is a condition in which a person has an enduring tendency to develop seizures. Seizures may involve convulsions, loss of awareness, staring, unusual sensations or brief repetitive movements. The type of seizure and its cause guide treatment.
A first suspected seizure needs clinical assessment to determine what happened and whether there was a provoking factor. EEG, blood tests and brain imaging may be used, but the investigations are selected according to the circumstances. One seizure does not always mean epilepsy.
Epilepsy is considered drug-resistant when adequate trials of two appropriate and tolerated anti-seizure medicine schedules fail to achieve sustained seizure freedom. Specialist evaluation can consider whether surgery, stimulation-based treatment, dietary therapy or another approach may help.
A convulsive seizure lasting five minutes or repeated seizures without recovery is a medical emergency. Prompt treatment reduces the risk of complications.
This infection develops when Taenia solium eggs are swallowed through fecal contamination of food, water or hands. Cysts in the nervous system can cause seizures, headache or raised pressure within the skull. Treatment depends on the number, location and stage of the lesions and the symptoms present.
Parkinson's can cause slowness, stiffness, tremor and balance problems, as well as changes in sleep, mood, bowel function or thinking. Medicines, physical activity and therapy are adjusted as symptoms change. Deep brain stimulation may help selected patients, but it does not cure the condition or stop progression.
This usually causes shaking during actions such as writing, eating or holding a cup. It differs from Parkinson's disease, although a neurological examination may be needed to distinguish them.
Dystonia causes involuntary muscle contractions and abnormal postures or movements. Treatment depends on the body area involved and may include medicines, botulinum toxin injections or surgery in selected cases.
Damage to peripheral nerves can cause numbness, burning, pain or weakness, often beginning in the feet. Diabetes is a common cause, but vitamin deficiencies, immune conditions, infections, medicines and inherited disorders may also be responsible.
This immune-mediated condition often causes weakness that progresses over days, sometimes after an infection. It can affect breathing, swallowing or heart rate and requires urgent hospital assessment.
Muscle weakness typically worsens with activity and may affect the eyelids, eye movements, speech, swallowing or breathing. Sudden breathing or swallowing difficulty is an emergency.
  • Motor neuron disease and muscular disorders: Progressive weakness can arise from disorders affecting motor neurons, muscles or the nerve-muscle connection. Diagnosis may require imaging, nerve and muscle testing, blood tests or genetic evaluation.
Carpal tunnel syndrome and other compression neuropathies can cause pain, tingling, numbness or weakness in a specific nerve distribution. Treatment ranges from activity changes and splinting to injections or surgery.
Tumors within the skull may be benign, malignant, primary or metastatic. Symptoms depend on their location and can include seizures, persistent or changing headache, weakness, changes in behavior, visual symptoms or problems with balance. Imaging suggests the diagnosis, but tissue testing is often needed to identify the tumor type.
Pituitary tumors may cause hormone-related symptoms, visual field loss or headache. Some can be treated with medicine, while others need surgery or radiotherapy depending on the tumor type, size and effect on nearby structures.
  • Spinal tumors and metastases: Tumors within or around the spinal canal may cause persistent pain, weakness, numbness or bladder and bowel problems. New neurological symptoms with back pain require urgent assessment because delayed treatment can lead to permanent loss of function.
Spine and Spinal Cord Conditions
A disc can irritate or compress a nerve root, causing neck or back pain that travels into an arm or leg. Many cases improve without surgery, but progressive weakness or severe persistent symptoms may require surgical assessment.
  • Cervical or lumbar spondylosis: Age-related changes in discs and joints can cause pain and stiffness and may narrow the spaces available for nerves or the spinal cord.
  • Spinal canal stenosis: Narrowing of the spinal canal can cause pain, heaviness, numbness or weakness when standing or walking. Treatment depends on symptom severity, neurological findings and imaging.
  • Spinal cord or cauda equina compression: Pressure from a disc, tumor, infection, bleeding or fracture can cause progressive weakness, altered sensation or loss of bladder or bowel control. This is a medical emergency and may require urgent surgery.
Infection of a vertebra or disc can cause persistent pain, fever, weight loss or neurological weakness. Tuberculosis is one possible cause in India, but bacterial and other infections must also be considered.
Injury, Infection and Cognitive Conditions
Head injury ranges from concussion to bleeding or swelling inside the skull. Repeated vomiting, increasing drowsiness, confusion, seizure, weakness, unequal pupils or worsening headache after an injury needs emergency assessment.
  • Subdural and extradural hematoma: Blood can collect between the brain and skull after injury. Some bleeds cause rapid deterioration, while chronic subdural hematoma may develop gradually, particularly in older adults or people taking blood-thinning medicines.
Excess cerebrospinal fluid can enlarge the spaces within the brain and raise pressure. Symptoms vary with age and cause and may include headache, vomiting, drowsiness, walking difficulty, cognitive change or urinary symptoms.
  • Meningitis, encephalitis and brain abscess: Infection or inflammation involving the brain or its coverings can cause fever, severe headache, neck stiffness, confusion, seizures or neurological weakness. These conditions require urgent assessment and treatment.
  • Multiple sclerosis and autoimmune neurological disease: Immune-mediated conditions can cause episodes of visual loss, weakness, numbness, imbalance, seizures or behavioral change. Diagnosis combines the clinical pattern with imaging, laboratory tests and, in selected cases, cerebrospinal fluid analysis.
  • Memory and cognitive disorders: Memory loss can arise from neurodegenerative disease, stroke, sleep problems, depression, medication effects, thyroid disease, vitamin deficiency or other conditions. Assessment looks for potentially treatable contributors as well as longer-term neurological causes.
  • Trigeminal neuralgia and facial weakness: Trigeminal neuralgia causes brief, electric-shock-like facial pain, while facial palsy causes weakness on one side of the face. The diagnosis determines whether treatment involves medicine, rehabilitation, a procedure or surgery.
  • Vertigo and balance disorders: Dizziness and imbalance may come from the inner ear, brain, circulation, medicines or other causes. Sudden vertigo with weakness, double vision, severe headache or difficulty speaking needs emergency assessment.

The assessment includes the symptom pattern, time of onset, medical and family history, medicines and a focused examination of movement, sensation, coordination, reflexes, speech, vision and cognition.

MRI of the brain or spine

MRI uses a magnetic field rather than ionizing radiation and provides detailed images of the brain, spinal cord, discs and nerves. Scan duration and the need for contrast depend on the clinical question. Patients must tell the imaging team about implants, pacemakers, metal fragments or previous procedures because each device requires an MRI safety check.

CT scan of the head or spine

CT is fast and is commonly used in emergencies to assess bleeding, fractures, hydrocephalus and other acute abnormalities. A normal early CT does not exclude every type of stroke, so further imaging may be required

These tests create images of blood vessels and may identify arterial blockage, narrowing, aneurysms or vascular malformations. Contrast use is planned according to the patient and the examination.

DSA uses a catheter and contrast to examine blood vessels in detail. It may be used when non-invasive imaging is insufficient or when a catheter-based treatment is being considered.

An EEG records electrical activity from sensors placed on the scalp. It may support the evaluation of seizures or altered awareness, but a normal routine EEG does not rule out epilepsy.

Continuous EEG recorded with synchronized video can capture a patient's typical events and help distinguish epileptic seizures from other episodes.

Nerve conduction study and electromyography

Nerve conduction testing uses brief electrical stimulation, while EMG records activity from selected muscles using a fine needle. These tests help assess neuropathy, nerve compression, motor neuron disorders and muscle disease.

A sample of cerebrospinal fluid may be collected from the lower back under sterile conditions and local anesthesia. It can help diagnose infection, inflammation, bleeding and other neurological conditions. Risks, including post-procedure headache, are discussed before the test.

Carotid Doppler examines blood flow in neck arteries, while transcranial Doppler assesses selected brain vessels. These are used when the clinical question and suspected condition make them appropriate.

Structured testing examines memory, attention, language, processing speed and problem-solving. Results are interpreted alongside education, mood, sleep, sensory function and daily abilities.

Blood, antibody and genetic testing

Tests may be used to identify metabolic, nutritional, infectious, autoimmune or inherited causes of neurological symptoms. The panel is chosen according to the clinical findings rather than ordered routinely for every patient.

Brain, nerve or muscle tissue may be sampled when imaging and other investigations cannot establish the diagnosis or when the exact tissue type will guide treatment.

Sudden facial drooping, weakness or numbness on one side, difficulty speaking, loss of balance or sudden visual loss may indicate stroke. Note when the person was last known to be well, call 112 and go to an emergency department immediately. Do not wait for the symptoms to settle.

2 Can stroke still be treated several hours after symptoms begin?

Possibly. Intravenous clot-dissolving treatment is generally considered within 4.5 hours for eligible patients, and selected patients may qualify beyond standard windows based on advanced imaging. Mechanical thrombectomy can benefit some large-vessel strokes for up to 24 hours after the person was last known to be well. Earlier arrival still provides the widest range of options.

A neurologist diagnoses and treats nervous-system conditions mainly through medicines and non-surgical care. A neurosurgeon performs operations involving the brain, spine and nerves. They may assess a patient together when the best approach is uncertain or several treatments are possible.

4 Are most headaches caused by a brain tumor?

No. Primary headache disorders such as migraine and tension-type headache are much more common. Urgent assessment is needed for a sudden severe headache, headache with fever and neck stiffness, new weakness or confusion, seizure, visual loss or a rapidly changing pattern.

5 Is MRI painful, and can everyone have one?

MRI is painless and does not use ionizing radiation, but the scanner is noisy and enclosed. Every pacemaker, implant, metal fragment or device must be checked for MRI safety. Some are MRI conditional and require specific precautions. Sedation may be considered only after appropriate assessment and monitoring arrangements.

6 Can a normal EEG rule out epilepsy?

No. A routine EEG records brain activity only during the test, so it may be normal between seizures. The history remains central, and repeat, sleep-deprived or prolonged video EEG may be useful when the diagnosis is uncertain.

7 What should I do if someone has a convulsive seizure?

Move dangerous objects away, place something soft under the head and time the seizure. Do not restrain the person or put anything in the mouth. Turn them onto their side when the movements stop and stay with them. Call emergency services if the seizure lasts five minutes, another begins without recovery, breathing is difficult, injury occurs or it is the first known seizure.

Many people achieve seizure freedom with appropriate treatment, but some continue to have seizures despite medicine. If two suitable anti-seizure medicine schedules have failed, referral to an epilepsy specialist is appropriate because surgery or another therapy may be considered.

9 Can a person with epilepsy work, drive or plan a pregnancy?

Many people with epilepsy work and have healthy pregnancies. Safety-sensitive work and driving depend on seizure control and applicable legal requirements. Pregnancy should be planned with the neurologist because medicine choice and dose may need review before conception. Anti-seizure medicine should never be stopped without medical advice.

There is currently no cure, but medicines, physical activity and therapy can improve symptoms and function. Deep brain stimulation may help selected patients whose symptoms or medication-related fluctuations are difficult to control, but it does not stop the disease from progressing.

11 Does sciatica always need spine surgery?

No. Many cases improve with activity modification, pain management and physiotherapy. Surgical assessment becomes more important when pain remains severe despite treatment or when there is progressive weakness. New bladder or bowel difficulty, numbness around the groin or rapidly worsening weakness requires emergency care.

No. Brain tumors include benign and malignant types, and cancer from another organ can also spread to the brain. Imaging provides important clues, but tissue examination is often needed to identify the exact type and guide treatment.

13 Can every brain tumor be completely removed?

No. The surgical goal depends on the tumor type, location and the neurological functions nearby. Sometimes complete removal can be attempted safely. In other cases, biopsy, partial removal or non-surgical treatment may offer a better balance of benefit and risk.

14 Will brain surgery affect memory, speech or personality?

The risk depends on the area of the brain involved and the planned procedure. Imaging, navigation, monitoring and awake mapping may help protect important functions in selected cases, but they cannot remove all risk. The surgeon should explain the individual risks before consent.

15 How long does recovery after stroke take?

Recovery varies widely with the size and location of the stroke, complications, previous health and access to rehabilitation. Improvement is often fastest in the first weeks and months but may continue for much longer. Therapy goals are reviewed as the person progresses.

16 Is memory loss always dementia?

No. Sleep problems, depression, hearing loss, thyroid disease, vitamin deficiency, medication effects, infection and other conditions can affect memory. Assessment combines the history, cognitive testing and selected investigations to identify treatable contributors and determine whether a neurodegenerative condition is present.
