Lymphedema Management
Lymphedema is a common yet under-recognised condition that can develop after cancer surgery, particularly following lymph node removal or radiation therapy. Most frequently seen after breast cancer surgery, lymphedema results from impaired lymphatic drainage, leading to persistent swelling, discomfort, restricted mobility, and an increased risk of infection.
Common Symptoms of Lymphedema
- Swelling, typically in the arm, leg, or breast on the side of surgery
- Heaviness/tightness in the affected limb
- Hardening (Fibrosis)
- Skin may become firm or leathery if left untreated
- Pitting edema, where pressing the skin leaves an indentation
- Cellulitis and skin infections due to fluid stagnation
- Dryness, thickening, peeling, or an orange-peel-like appearance of the skin
Cancer Surgeries Most Commonly Associated with Lymphedema
- Breast cancer (after axillary lymph node dissection or sentinel node biopsy)
- Cervical, endometrial, and vulvar cancer
- Melanoma (especially if groin or axillary nodes removed)

- Lymphovenous Anastomosis (LVA)
For early-stage lymphedema, LVA offers a minimally invasive surgical solution. This microsurgical procedure connects small lymphatic vessels directly to nearby veins, allowing lymph fluid to bypass the blockage and drain into the venous system.
- Lymph Node Transfer (LNT)
In more advanced or chronic cases, LNT involves transferring healthy lymph nodes from one part of the body to the affected area to rebuild functional drainage pathways. Over time, this can significantly reduce limb swelling and improve lymphatic function.
- Liposuction for Lymphedema
When lymphedema leads to permanent fibrotic tissue changes, liposuction may be used to remove excess fat and protein-rich fluid. This procedure reduces the volume and weight of the limb, enhances contour, and relieves physical strain.
- Prophylactic Surgery
For high-risk patients, particularly those undergoing axillary clearance, preventive LVA may be performed at the time of cancer surgery. This proactive measure helps reduce the likelihood of developing lymphedema post-operatively.
- Manual Lymphatic Drainage (MLD)
A gentle, hands-on therapy performed by certified physiotherapists, MLD helps stimulate lymph flow, reduce inflammation, soften fibrotic tissue, and improve overall comfort.
- Compression Therapy
Custom-fitted compression garments are used to reduce swelling, enhance lymphatic flow, and maintain limb shape. These garments play a vital role in day-to-day symptom control and in preventing progression.
- Targeted Exercise Programs
Structured movement regimens are tailored to promote muscle activity, which naturally supports lymphatic circulation. Exercises also help strengthen the affected limb, improve flexibility, and enhance overall physical function.
