Types Of 
Vaginal Cancer
There are different types of vaginal cancer, named based on where the cancer begins:

  • Begins in the flat cells that line the vagina (squamous cells); it is the most common type of vaginal cancer and accounts for nearly 90% of all cases

  • Begins in gland cells in the vagina; it’s most common in people over 50

  • Begins in the cells that give the vagina its colour (melanocytes). Vaginal melanomas are extremely rare

  • Begins in the connective tissue and muscle tissue that make up the vaginal wall. This type is extremely rare

Staging of Vaginal Cancer

Vaginal cancer is staged using the TNM system to describe the cancer's size and spread. The prognosis for vaginal cancer depends on the stage at diagnosis, with 5-year survival rates of 76% for localized, 59% for regional, and 24% for distant stages.

Stage I

Cancer is limited to the vaginal wall.

Stage II

Cancer has grown into the subvaginal tissue but has not extended to the pelvic wall.

Stage III

Cancer has extended to the pelvic wall.

Stage IV

Cancer has spread to nearby organs, such as the bladder or rectum, or has spread outside the true pelvis.

Surgical Oncology

  • Wide local excision for small, early-stage lesions
  • Partial or total vaginectomy (removal of part or all of the vagina)
  • Lymph node dissection (inguinal or pelvic, if needed)
  • Reconstructive surgery may be offered post-excision to restore anatomy and function
  • Palliative surgery to relieve symptoms in advanced cases

Minimally invasive and robotic-assisted options are considered when appropriate to reduce recovery time and improve outcomes.

Radiation Oncology

Radiation therapy is a mainstay for vaginal cancer, especially for locally advanced stages:

  • External beam radiation therapy (EBRT) targets the tumor and regional lymph nodes.
  • Intracavity brachytherapy delivers internal radiation directly into the vaginal tissues.
  • Chemoradiation may be used in high-grade or locally advanced cases.
