Types Of 
Vulvar Cancer
Based on the type of cells where the cancer starts, the most common types of vulvar cancer include:

The most common type, accounting for around 90% of all cases. Begins in the squamous cells on the surface of the vulvar skin.

Represents approximately 5% of vulvar cancers. A fast-growing cancer arising from pigment-producing melanocytes.

The remaining vulvar cancers are rare, including basal cell carcinoma, Bartholin gland adenocarcinoma, Paget disease of the vulva, sarcoma and verrucous carcinoma.

Diagnostics

Staging of Vulvar Cancer

Vulvar cancer staging uses the FIGO system. The prognosis depends on its stage, with 5-year survival rates being 86% for localized cancer, 53% if it has spread to nearby lymph nodes, and 19% for distant metastasis.

Stage 0

Cancerous cells are on the surface of the vulvar skin only, also known as vulvar intraepithelial neoplasia

Stage I

A cancerous tumor is found on the vulva but has not spread.

Stage II

The tumor is larger than 2 cm and has spread to nearby tissues or the perineum, but not to the lymph nodes.

Stage III

The cancer has spread to nearby tissues and/or one-sided lymph nodes, but not to distant parts of the body.

Stage IV

Cancer has spread to distant parts of the body or to lymph nodes on both sides.

  • Sentinel lymph node biopsy for nodal assessment

  • Palliative surgery may be considered to relieve symptoms

Radiation Oncology

Radiation therapy is an important tool in vulvar cancer care, used in various settings:

  • Neoadjuvant radiation may be used to shrink large tumors and make surgery less extensive.
  • Adjuvant radiation is recommended if cancer cells are found in lymph nodes or if margins are close/positive.
  • Concurrent chemoradiation (using low-dose chemotherapy during radiation) may enhance effectiveness in advanced cases.
  • Radiation to lymph node regions may help eliminate microscopic disease and prevent recurrence.
