Types Of Breast Cancer

Breast cancer is classified based on where it originates and its characteristics:

  • Ductal Carcinoma In Situ (DCIS): Non-invasive, confined to the milk ducts.
  • Invasive Ductal Carcinoma (IDC): The most common type; starts in ducts and spreads to surrounding tissue.

  • Lobular Carcinoma In Situ (LCIS): A marker of increased risk, not true cancer.
  • Invasive Lobular Carcinoma (ILC): Starts in the lobules and can spread.

  • Triple-Negative Breast Cancer: Lacks estrogen, progesterone, and HER2 receptors.
  • HER2-Positive Breast Cancer: Overexpresses HER2 protein, grows aggressively.
  • Inflammatory Breast Cancer: A rare, aggressive type causing swelling/redness.
  • Paget's Disease of the Nipple: Involves nipple and areola skin.

Breast Cancer & Hormone Receptor Status

In the context of breast cancer, hormone receptors refer to specific proteins found on the surface or inside of breast cancer cells that can bind to hormones; mainly estrogen and progesterone. These hormones can stimulate the growth of some breast cancer cells. 
Hormone Receptor- 
Positive
The cancer cells have receptors for estrogen (ER+), progesterone (PR+), or both. These cancers depend on hormones to grow. They often respond well to hormone therapy, which blocks the effect of hormones or lowers hormone levels in the body. 
Hormone Receptor- 
Negative
The cancer cells do not have estrogen or progesterone receptors. These cancers do not respond to hormone-blocking treatments. It requires other forms of therapy like chemotherapy or targeted therapy.
HER2 
Positive
Not a hormone receptor but often tested alongside hormone receptors. These cancers overexpress the HER2 protein, making them grow faster. It requires targeted HER2 therapies.  
Triple- 
negative
The breast cancer cells lack receptors for estrogen, progesterone and HER2. This is a more aggressive cancer with fewer treatment options. It is best treated with chemotherapy or immunotherapy.

Staging of Breast Cancer

Breast cancer staging (TNM system) is based on tumour size, lymph node involvement, and metastasis. Prognosis is based on the stage. Early detection (Stage 0 or I) has a 5‑year survival rate of over 90%. Advanced stages (Stage III or IV) have lower survival rates, but new therapies have improved outcomes.

Radiation Oncology

  • External Beam Radiation Therapy (EBRT), standard modality with precise targeting
  • 3D Conformal and IMRT (Intensity-Modulated Radiation Therapy), ensures reduced dose to nearby organs
  • Hypofractionated Radiation; shorter-duration regimens proven to be as effective and safe as conventional courses
  • Proton Therapy; this advanced modality is particularly useful in left-sided breast cancer, post-mastectomy radiation, or in younger women where heart and lung preservation is critical. It delivers high-dose radiation to the tumor while minimizing dose and protecting nearby vital organs.
