What an abnormal LFT is

A liver panel reports several unrelated measurements together. Each behaves differently:

  • SGPT/ALT and SGOT/AST are enzymes released when liver cells are injured. AST also comes from muscle, heart and red blood cells, so AST can rise without liver disease.
  • ALP rises with bile duct problems, but also comes from bone, placenta and intestine. Growing children and pregnancy normally show higher ALP.
  • జిజిటి is sensitive but not specific. GGT rises with alcohol, several medicines and fatty liver, and a raised GGT alone rarely means much.
  • బిలిరుబిన్ rises with impaired bile handling, bile duct obstruction, or breakdown of red cells (haemolysis). Gilbert syndrome causes a harmless mild unconjugated rise, often more noticeable after fasting or illness.
  • Albumin and total protein reflect synthesis, but also nutrition, inflammation, kidney protein loss and gut loss.
  • PT/INR (often ordered separately) is one of the few genuine markers of liver synthetic function.

An "abnormal" flag on a report means the value sits outside that laboratory's reference interval. Reference intervals differ between laboratories by method, analyser and population, which is why the same blood can be flagged in one lab and not another. Always read the ranges printed on your own report rather than comparing with someone else's.

Why abnormal LFTs are investigated

  • To separate liver causes from non-liver causes such as muscle injury, thyroid disease, coeliac disease, heart failure or haemolysis.
  • To identify treatable causes: hepatitis B and C, autoimmune hepatitis, Wilson disease, haemochromatosis, drug-induced liver injury, bile duct obstruction.
  • To detect metabolic dysfunction-associated steatotic liver disease (MASLD), the current term replacing NAFLD, and to look for fibrosis, which is the finding that most affects long-term outcome.
  • To judge severity when jaundice, ascites or confusion are present.

Who should get a liver function test

  • Anyone with jaundice, dark urine, pale stools, persistent itching, right upper abdominal pain, unexplained nausea or marked fatigue.
  • People with abdominal swelling, easy bruising or vomiting of blood, urgently.
  • People with type 2 diabetes, insulin resistance, dyslipidaemia, hypertension or central weight gain, in whom steatotic liver disease is common. In South Asians, metabolic liver disease occurs at lower body mass index than in many other populations.
  • People on potentially hepatotoxic medicines when the prescriber advises monitoring, for example anti-tubercular therapy, methotrexate, some antiepileptics, antifungals, and certain antiretrovirals. Monitoring schedules are set by the prescriber.
  • People using AYUSH, herbal, weight-loss or gym supplements, which are a recognised and under-reported cause of drug-induced liver injury in India.
  • Regular or heavy alcohol users.
  • People with hepatitis B or C, or a family member with hepatitis B, and those with a family history of liver disease, Wilson disease or haemochromatosis.
  • People with known chronic liver disease, for periodic monitoring.

Routine LFT screening in a person with no symptoms and no risk factors often produces mildly abnormal results that lead to further testing without benefit. Discuss with a doctor whether the test answers a real question for you.

What an LFT cannot detect or exclude

  • An LFT does not give a diagnosis. A raised SGPT does not say whether the cause is fat, a virus, a medicine or an autoimmune process.
  • An LFT does not measure fibrosis or cirrhosis. People with established cirrhosis frequently have normal or near-normal enzymes. A normal LFT does not exclude advanced liver disease.
  • An LFT does not image the liver or bile ducts. Gallstones, bile duct stones, cysts, abscess and tumours are not visible on a blood panel.
  • An LFT does not screen for liver cancer. Surveillance in at-risk people uses imaging, with or without AFP, as advised by a hepatologist.
  • Enzyme height does not equal severity. Very high ALT can occur in a self-limiting viral hepatitis; near-normal ALT can accompany decompensated cirrhosis.
  • Normal albumin and bilirubin do not confirm good function in early disease, as these change late.
  • An LFT does not identify the specific virus. Hepatitis A, B, C and E require separate serological tests.

A result described as "not identified" or "within range" describes what was measured at that moment, it does not exclude disease. If symptoms persist despite a normal result, return to your doctor.

These are complementary, not ranked. Which one comes first depends on the presentation: painless jaundice with itching and a cholestatic pattern usually prompts imaging of the bile ducts early; a mildly raised SGPT with diabetes and no symptoms usually prompts repeat testing, medicine and alcohol review, hepatitis B and C testing and a fibrosis score before anything else.

Preparation is an operational matter and varies between laboratories. Your unit's written instructions take precedence over anything here.

  • Fasting is generally not needed for an LFT alone. Many Indian laboratories bundle LFT into a health package with lipids or fasting glucose, in which case fasting may be requested.
  • Bring a full list of medicines, including painkillers, antibiotics, antifungals, anti-tubercular drugs, and all herbal, AYUSH, ayurvedic, homeopathic and gym or weight-loss supplements.
  • Tell the laboratory about recent alcohol intake, recent vigorous exercise or gym work, recent illness and pregnancy.
  • Carry previous LFT reports. Trend over time is often more informative than one value.
  • Do not start, stop or change any prescribed medicine to "improve" the result. Speak to the prescriber.

If you are fasting and take glucose-lowering medicines

Prolonged fasting can cause hypoglycaemia in people on insulin or sulfonylureas. Ask the prescriber how to manage doses on a fasting morning, book an early slot, and carry a sugar source.

ALP rises normally in pregnancy because of placental production, so a raised ALP is often not a liver problem. Itching in later pregnancy, however, needs prompt assessment for intrahepatic cholestasis of pregnancy. Report new itching, jaundice, severe upper abdominal pain, headache or visual disturbance to your obstetrician without delay.

ALP is normally much higher in growing children and adolescents, and paediatric reference intervals differ by age. Paediatric results should be interpreted against age-specific ranges by a paediatrician.

Older adults, CKD and heart failure

Albumin falls with age, chronic inflammation, kidney protein loss and poor intake. In heart failure, hepatic congestion can raise enzymes and bilirubin without primary liver disease. If you have a fluid restriction, do not increase fluid intake before a blood test on generic advice.

A phlebotomist cleans the skin, applies a tourniquet and draws blood, usually from a vein in the arm. The draw takes a few minutes. Mild pain, brief bruising or a small haematoma can occur; fainting is uncommon but possible, so mention it if you have fainted at blood tests before. Pressure over the site for a minute or two reduces bruising. Most people resume normal activity immediately. People on blood thinners should hold pressure longer and should mention the medicine to the phlebotomist.

Read your own report's reference ranges. Numbers are not comparable across laboratories. The useful questions are: which group of values is raised, by how much, and is it changing?

Hepatocellular pattern

SGPT/ALT and SGOT/AST are raised out of proportion to ALP. Common associations include steatotic liver disease, alcohol, medicines and supplements, viral hepatitis (including hepatitis E, which is common in India), autoimmune hepatitis, coeliac disease, thyroid disorders and Wilson disease in younger patients.

ALP and GGT are raised out of proportion to the transaminases, often with raised bilirubin, dark urine, pale stools and itching. Causes include bile duct stones, strictures, tumours compressing the duct, drug-induced cholestasis and primary biliary cholangitis. This pattern usually leads to imaging of the biliary tree.

Isolated raised bilirubin

If bilirubin is mildly raised, mostly unconjugated, and all other values are normal in a well person, Gilbert syndrome or mild haemolysis are common explanations. Haemolysis is assessed with additional tests such as reticulocyte count and LDH.

Isolated raised ALP or GGT

A lone ALP rise may be of bone or placental origin. A lone GGT rise is very non-specific and frequently needs no action beyond review of alcohol and medicines, and repeat testing.

When LFT results can be misleading

Falsely raised or non-hepatic elevation

  • AST: strenuous exercise, gym weight training, intramuscular injections, muscle disease, haemolysis in the sample, and heart or skeletal muscle injury can raise AST with a healthy liver.
  • ALP: pregnancy (placental), childhood and adolescent growth, healing fractures and bone disease.

  • GGT: recent alcohol, several common medicines, obesity, often with no clinically important liver disease.
  • Delayed transport or a haemolysed sample can raise AST, bilirubin and potassium spuriously; repeat sampling resolves the doubt.

Falsely reassuring or falsely lowered

  • Normal enzymes in advanced disease: in cirrhosis, enzyme levels often fall towards normal as functioning liver tissue is lost.
  • Normal ALT in significant MASLD with fibrosis is common, so a normal LFT is not clearance.
  • AST/ALT lowered by vitamin B6 deficiency is described, and can blunt expected enzyme rises.
  • Single-point testing: enzyme levels fluctuate in chronic hepatitis C and autoimmune hepatitis, so a normal reading on one day may miss activity.
  • Bilirubin can be normal in early bile duct obstruction and in many chronic liver diseases.

The blood test

Venepuncture carries small risks: pain, bruising, haematoma, light-headedness, and rarely local infection or nerve irritation. These are usually minor and short-lived. People on anticoagulants or with bleeding disorders should tell staff beforehand.

Downstream imaging

Ultrasound uses no ionising radiation. CT involves radiation exposure and iodinated contrast may be unsuitable for some people with kidney impairment or prior contrast reactions. MRI requires a safety assessment for implants, pacemakers and metal fragments. Each of these has a different risk profile and is decided case by case.

Causes of abnormal LFTs

  • Steatotic liver disease (SLD): the umbrella term introduced in the 2023 multi-society Delphi consensus (Rinella et al.). MASLD replaces NAFLD; MASH replaces NASH; MetALD describes metabolic dysfunction-associated steatotic liver disease with a significant alcohol contribution. Alcohol-related liver disease is a separate category.

How abnormal LFTs are assessed and treated

Management targets the cause, not the number. A typical pathway includes a careful history of alcohol, medicines and supplements; examination; repeat LFT to confirm; hepatitis B and C testing; metabolic assessment including glucose, HbA1c and lipids; thyroid and coeliac testing where relevant; USG abdomen; and fibrosis assessment with a score such as FIB-4 followed by elastography where indicated. Autoimmune antibodies, iron studies, copper studies and caeruloplasmin are added when the pattern suggests them. Biopsy is used selectively.

Treatment follows the diagnosis: antiviral therapy for chronic hepatitis B or C, immunosuppression for autoimmune hepatitis, chelation for Wilson disease, venesection for haemochromatosis, biliary drainage or surgery for obstruction, withdrawal of a culprit drug or supplement under medical guidance, alcohol cessation with structured support where alcohol is contributing, and for MASLD a combination of gradual sustained lifestyle change, treatment of diabetes and lipids, and newer liver-directed drug therapy in selected patients under specialist care. Advanced cirrhosis with liver failure may lead to transplant assessment. Outcomes differ widely between individuals and between causes.

Reducing avoidable liver injury

  • Vaccination against hepatitis B, and hepatitis A where advised.
  • Safe drinking water and food hygiene reduce hepatitis A and E exposure.
  • Check with a doctor or pharmacist before starting any supplement, herbal product or over-the-counter painkiller, particularly if you have liver disease.
  • Keep within the paracetamol dose your doctor advises, and be aware that combination cold remedies also contain paracetamol.
  • Regular physical activity, treatment of diabetes and lipids, and reducing alcohol intake all reduce progression risk in steatotic liver disease.
  • Attend monitoring blood tests advised during anti-tubercular or other hepatotoxic therapy.

Abnormal LFTs in the Indian context

Typical next steps are a medicine and supplement review, an alcohol history, a repeat test after a few weeks avoiding heavy exercise beforehand, hepatitis B and C testing, metabolic assessment, and a fibrosis score. Your doctor decides the sequence based on your risk factors.

Can exercise or gym supplements raise liver enzymes?

Yes. Strenuous resistance training can raise AST and, less so, ALT from muscle rather than liver. Separately, several bodybuilding and weight-loss supplements have caused genuine liver injury. Tell your doctor about all products used, ideally bringing the packet, and mention recent heavy workouts before testing.

Does a raised bilirubin always mean jaundice or liver disease?

No. A mildly raised unconjugated bilirubin with otherwise normal values in a well person often reflects Gilbert syndrome, a harmless inherited variation that becomes more obvious with fasting or illness. Mild haemolysis is another explanation. Visible yellowing usually needs a higher bilirubin level and warrants prompt medical review.

Can abnormal LFTs return to normal?

Often, yes, when a reversible cause such as a medicine, supplement, alcohol or an acute viral hepatitis is addressed, and frequently with sustained metabolic improvement in MASLD. Established cirrhosis is not reversed, though progression can often be slowed. Results vary depending on individual clinical circumstances.

Is an ultrasound better than an LFT?

Neither is superior; they answer different questions. The LFT shows whether there is biochemical injury or obstruction and in what pattern. Ultrasound shows structure, fat, size, nodularity, gallstones, duct dilatation. Ultrasound cannot assess inflammation and can miss mild steatosis and small bile duct stones.

Should my family be tested if I have hepatitis B?

Household members and sexual partners of a person with hepatitis B are usually advised to be tested and, if not immune, vaccinated. Newborns of mothers with hepatitis B require specific timely measures. Discuss testing and vaccination plans with the treating physician.
