Beta Thalassaemia Test: Screening, Diagnosis and Understanding Your Report

19 оны хоёрдугаар сарын 2025
Хуваалцах арга:

A beta thalassaemia test is a group of blood tests, usually a complete blood count (CBC) with red cell indices, followed by haemoglobin (Hb) HPLC or electrophoresis, and where needed DNA analysis, used to identify beta thalassaemia trait, intermedia or major. The tests measure HbA2, HbF and abnormal haemoglobin variants, and require interpretation alongside iron status and clinical context.

Түлхүүрийг авах

  • Beta thalassaemia testing usually starts with a CBC; a low MCH or MCV raises suspicion but does not by itself distinguish thalassaemia trait from iron deficiency.
  • Haemoglobin HPLC (or electrophoresis) quantifies HbA2 and HbF, and is the main test used to identify beta thalassaemia trait.
  • A raised HbA2 supports beta thalassaemia trait, but iron deficiency, severe anaemia and recent transfusion can distort HbA2 and HbF values in either direction.
  • DNA mutation analysis confirms the specific beta-globin gene change and is used for couple screening, prenatal diagnosis and unclear HPLC patterns.
  • A normal HPLC result does not exclude every thalassaemia or haemoglobinopathy, alpha thalassaemia and some silent beta variants can be missed.

What the beta thalassaemia test is

Beta thalassaemia is an inherited condition in which the body makes less beta-globin, one of the protein chains of adult haemoglobin. Less beta-globin means smaller, paler red cells and, depending on the severity, anaemia ranging from negligible to transfusion-dependent.

No single test is "the" beta thalassaemia test. In practice, a stepwise panel is used:

  • Complete blood count (CBC) with red cell indices: looks at haemoglobin, MCV, MCH, red cell count and RDW. In beta thalassaemia trait the red cells are typically small and pale while the red cell count is relatively preserved, but this pattern overlaps with iron deficiency.
  • Захын цусны түрхэц: a microscopist describes red cell shape, target cells and basophilic stippling. The smear is supportive, not diagnostic.
  • Haemoglobin HPLC or capillary electrophoresis: separates and quantifies haemoglobin fractions, HbA, HbA2, HbF, and detects variants such as HbE, HbS and HbD. A raised HbA2 fraction is the usual laboratory signature of beta thalassaemia trait.
  • Iron studies (commonly serum ferritin): run alongside, because iron deficiency is extremely common in India and can lower HbA2 enough to hide a trait.
  • HBB gene (DNA) analysis: identifies the exact mutation. Used for confirming ambiguous results, for at-risk couples, and for prenatal diagnosis.

Why the beta thalassaemia test is done

  • To find the cause of persistent microcytic, hypochromic anaemia that has not responded to iron therapy.
  • To identify carriers (beta thalassaemia trait) before or during pregnancy, so that couple risk can be assessed.
  • To confirm a suspected diagnosis in a child with anaemia, poor growth, jaundice or an enlarged spleen.
  • To characterise the exact mutation when this affects counselling or prenatal testing.
  • To screen relatives of a person already diagnosed with thalassaemia or a haemoglobin variant.

Энэ шинжилгээг хэн өгөх ёстой вэ

  • Anyone with unexplained low MCV or MCH on a routine CBC, particularly if iron studies are normal or anaemia persists despite iron supplementation.
  • Couples planning marriage or pregnancy, especially where there is consanguinity or a family history of anaemia or transfusion dependence.
  • Pregnant women at the first antenatal visit, with partner testing if the woman is found to be a carrier. Testing early in pregnancy matters because prenatal diagnosis is time-sensitive.
  • Нэгдүгээр зэрэглэлийн хамаатан садан: parents, siblings, children, of a person with thalassaemia trait, thalassaemia major or a haemoglobin variant such as HbE or HbS.
  • Children with pallor, poor weight gain, recurrent infections, jaundice or splenomegaly, typically becoming apparent from around six months of age in severe beta thalassaemia.
  • Communities with higher carrier frequency in India, including several Sindhi, Punjabi, Gujarati, Bengali, Mahar, Kolli, Saraswat, Lohana and Gaur groups, and tribal populations where HbE and HbS are also common.

Whether you need testing is a clinical decision. Speak to your doctor rather than ordering a panel on your own, because the right first test depends on why the question is being asked.

What the beta thalassaemia test cannot detect or exclude

This is the part most often left out of test descriptions, and it matters.

  • A normal Hb HPLC does not exclude all thalassaemias. Alpha thalassaemia trait usually produces small red cells with a Хэвийн HbA2 and is not diagnosed by HPLC. It needs DNA testing for alpha-globin genes.
  • Silent and borderline beta variants can be missed. Some beta mutations produce only a borderline HbA2 rise; these may not be identified without DNA analysis.
  • HPLC cannot tell you how severe the disease will be. Genotype predicts phenotype only imperfectly. Two people with the same mutation pair can have different transfusion requirements. Results vary depending on individual clinical circumstances.
  • The test does not assess iron overload, organ damage or cardiac function. Ferritin, MRI T2* and echocardiography answer those questions separately.
  • Carrier testing after a recent transfusion is unreliable, because the donor's haemoglobin is being measured alongside the patient's.
  • Targeted DNA panels only look for the mutations they are designed to find. A "no mutation detected" report means the tested mutations were not identified in that sample, it is not the same as saying no beta-globin abnormality exists.

If a result is normal but symptoms such as fatigue, breathlessness or persistent anaemia continue, go back to your doctor. A negative test describes what was seen, not what is impossible.

туршилтынАсуулт нь хариулдагЮу хийдэггүй вэ
Улаан эсийн индекс бүхий CBCIs there anaemia? Are the red cells small and pale?Cannot distinguish thalassaemia trait from iron deficiency on its own
Serum ferritin and iron studiesIs iron deficiency present or contributing?Says nothing about globin gene status; ferritin rises in infection and inflammation
Hb HPLC / capillary electrophoresisWhat proportions of HbA, HbA2, HbF and variant haemoglobins are present?Does not diagnose alpha thalassaemia; unreliable after transfusion
HBB gene (DNA) analysisWhich specific beta-globin mutation is present?Does not predict severity precisely; limited to mutations covered by the method used
Alpha-globin gene analysisIs alpha thalassaemia the cause of microcytosis?Separate test; not included in a standard thalassaemia HPLC profile
Newborn screening (cord or heel-prick)Is a significant haemoglobin disorder likely in this baby?HbF is high at birth, so beta thalassaemia trait is not reliably identified in newborns
Prenatal diagnosis (CVS or amniocentesis with DNA testing)Has the foetus inherited two beta thalassaemia mutations?Invasive, time-limited by gestation, and offered only after both parents are characterised
Sickle solubility testIs sickle haemoglobin present?Does not quantify HbA2 or diagnose beta thalassaemia; cannot separate sickle trait from sickle disease

Which test comes first depends on the clinical problem. For a tired adult with microcytic anaemia and no family history, iron studies plus CBC often come first. For a couple in early pregnancy with a family history, CBC and HPLC for both partners are the priority, because the timeline for prenatal options is short.

  • Мацаг барих нь ихэвчлэн шаардлагагүй байдаг for CBC or Hb HPLC. If iron studies or other tests are bundled in, your laboratory may ask you to fast or to give the sample in the morning. Follow the instructions your unit gives you, these are operational protocols and vary between laboratories.
  • Tell the laboratory and your doctor about any blood transfusion in the previous three to four months. HPLC results in that window can be misleading, and testing may need to be deferred or interpreted differently.
  • Report iron supplements, folic acid, hydroxyurea or recent treatment for anaemia. Do not stop any prescribed medicine to prepare for this test, speak to the prescriber first.
  • Bring previous reports, including old CBCs and any family member's thalassaemia report. Comparison is genuinely useful.
  • Carry a family history: anaemia, transfusions, early deaths from anaemia, or a relative labelled a "carrier".

Tell the phlebotomist you are pregnant. Testing is usually arranged as early in pregnancy as possible so that partner testing and, if needed, prenatal diagnosis can be discussed within the available window. Physiological changes in pregnancy, and iron deficiency which is common in pregnancy, can affect interpretation.

If your child is being tested

In infants, HbF is naturally high and falls over the first year, so HbA2-based carrier diagnosis is generally unreliable in the first few months of life. Your paediatrician may advise repeating the test later. Bring feeding and growth records if available.

If you have had, or are due, a transfusion

Wherever possible, a diagnostic sample is taken өмнө a transfusion. If you have already been transfused, tell the treating team, DNA testing may be preferred over HPLC.

If you have another chronic condition

Chronic kidney disease, inflammatory disease, liver disease and vitamin B12 or folate deficiency all alter red cell indices and can complicate interpretation. Mention these conditions when the test is requested.

A phlebotomist cleans the skin, applies a tourniquet and draws blood from a vein in the arm, usually into an EDTA tube. For infants, a heel-prick or a small venous sample may be used. Most people feel a brief sting. Pressure on the site for a minute or two reduces bruising.

If DNA analysis is planned, the same or an additional EDTA sample is generally sufficient, a separate visit is not usually needed.

  • Цус цуглуулах: хэдэн минут.
  • Total time at the laboratory: commonly well under an hour, mostly registration and waiting.
  • Тайлагнах: CBC is usually the quickest; Hb HPLC takes longer; DNA mutation analysis and prenatal diagnosis take substantially longer because they involve specialised molecular workflows and often a referral laboratory. Ask your laboratory for its own reporting timeline, as this varies between units.
  • The overall diagnostic pathway: CBC, iron studies, HPLC, partner testing and, if required, DNA analysis, can span several visits over some weeks. For couples in pregnancy, tell the team you are pregnant so the pathway can be expedited.

Таны тайланг ойлгох

Your report will typically list haemoglobin, MCV, MCH, RDW and red cell count, then the haemoglobin fractions from HPLC, with the laboratory's own reference intervals printed alongside. Reference intervals differ between laboratories and analysers, so always read the values against the ranges printed on your own report rather than against numbers found online.

What the doctor is looking at:

  • Улаан эсийн индексүүд: small (low MCV) and pale (low MCH) red cells with a near-normal or high red cell count and a relatively normal RDW is a pattern that raises suspicion of thalassaemia trait. Iron deficiency more often shows a raised RDW and a lower red cell count, but this is a tendency, not a rule, and the two conditions frequently coexist.
  • HbA2: an elevated HbA2 fraction is the usual finding in beta thalassaemia trait. Borderline values need repeat testing, iron correction or DNA analysis.
  • HbF: may be mildly raised in beta thalassaemia trait and markedly raised in thalassaemia major or intermedia. HbF is also naturally high in newborns and infants.
  • Variant peaks: HbE, HbS, HbD and others may appear. HbE-beta thalassaemia is an important combination in eastern and north-eastern India and can behave like thalassaemia major.
  • DNA report: names the mutation or mutations and whether they are in one copy of the gene (heterozygous, i.e. trait) or both copies (homozygous or compound heterozygous).

A report does not make a diagnosis by itself. Beta thalassaemia trait, thalassaemia intermedia and thalassaemia major are clinical diagnoses made by combining the laboratory pattern with your symptoms, transfusion history, examination and family testing. Take the report back to the doctor who requested it.

Results that can be falsely lowered or masked

  • Төмрийн дутагдал can reduce HbA2 and push a true carrier into the normal range. This is the single most important pitfall in India, where iron-deficiency anaemia is widespread. Repeat HPLC after iron status is corrected if suspicion remains.
  • Coexisting alpha thalassaemia can blunt the HbA2 rise in a beta carrier.
  • Саяхан цус сэлбүүлсэн dilutes the patient's own haemoglobin with donor HbA, lowering the apparent HbA2 and HbF fractions.
  • Severe vitamin B12 or folate deficiency alters red cell size and can obscure the classic microcytic pattern.

Results that can be falsely raised or over-called

  • Hyperthyroidism, some antiretroviral and antiepileptic drugs, and megaloblastic anaemia have been associated with raised HbA2 in the absence of beta thalassaemia.
  • HbE and some other variants can co-elute close to the HbA2 window on certain HPLC systems, inflating the reported HbA2. Laboratories usually flag this, but it is a known source of misreporting.
  • Hereditary persistence of foetal haemoglobin (HPFH) raises HbF without causing clinically significant disease and can be mistaken for thalassaemia.
  • Pregnancy and infancy alter baseline HbF, so age- and state-appropriate interpretation is essential.

If the laboratory pattern and the clinical picture do not match, the answer is usually to repeat the test, correct iron deficiency, test the parents, or move to DNA analysis, not to accept the first result at face value.

Blood draw is a low-risk procedure. Possible effects include brief pain, bruising, light-headedness or fainting, and rarely a small haematoma or local infection. Tell the phlebotomist if you have fainted during blood tests before, or if you take blood-thinning medicines, so pressure can be held for longer.

Heel-prick in infants

Brief discomfort and minor bruising are the usual issues. Repeat sampling is sometimes needed if the sample volume is inadequate.

Prenatal diagnostic sampling (CVS or amniocentesis)

These are invasive procedures with a small but real risk of miscarriage, bleeding, infection and, uncommonly, inconclusive results requiring repeat sampling. They are offered only after counselling, and the risk profile differs from a simple blood test, the two should not be described together as equally safe. Risks vary with gestation, operator experience and individual circumstances.

Genetic and psychosocial considerations

A carrier result has implications for siblings, children and reproductive planning. Genetic counselling before and after testing helps you understand what the result means for your family and what choices exist. Confidentiality of genetic information should be discussed with the testing centre.

Онцгой нөхцөл байдал

Жирэмсний

Carrier screening is most useful early. If the mother is a carrier, the partner is tested; if both are carriers, genetic counselling and the option of prenatal diagnosis are discussed. Iron deficiency in pregnancy can mask a trait, so interpretation must account for iron status. All prenatal testing in India is governed by the PC-PNDT Act, 1994, prenatal diagnosis for a genetic condition is permitted under the Act's provisions at registered centres with documented indication and consent, and sex determination is prohibited.

Нярай болон хүүхдүүд

HbF is physiologically high at birth, so newborn HPLC can suggest severe beta thalassaemia or a variant but does not reliably identify beta thalassaemia trait. Children with beta thalassaemia major usually present between six months and two years with pallor, poor growth and an enlarged spleen. Paediatric interpretation uses age-specific reference intervals.

Хуучин насанд хүрэгчид

Microcytic anaemia appearing for the first time in later life should not be attributed to thalassaemia without evaluation. Gastrointestinal blood loss and other causes of iron deficiency need to be considered, and a long-standing trait will usually be visible on old blood counts.

People with chronic kidney disease or chronic illness

Anaemia of chronic disease, renal anaemia and erythropoietin therapy all change red cell indices. These do not prevent HPLC from being done, but they make CBC-based screening less reliable.

Чихрийн шижин өвчтэй хүмүүс

Haemoglobin variants and thalassaemia can interfere with HbA1c measurement, sometimes producing falsely high or falsely low values depending on the method used. If you have a haemoglobin disorder and diabetes, tell your diabetes team so an appropriate monitoring method, for example, a variant-tolerant assay or alternative markers, can be chosen.

People who have had a splenectomy or regular transfusions

Interpretation shifts substantially. DNA testing is often the more dependable route in transfused patients, and monitoring focuses on iron overload and organ function rather than repeat HPLC.

Beta thalassaemia testing in the Indian context

India carries one of the largest burdens of beta thalassaemia in the world. Carrier frequency varies widely between regions and communities, and several distinct haemoglobinopathies overlap geographically, HbE in West Bengal, Assam and the North-East; HbS in central and western tribal belts; HbD in Punjab. This overlap is why a thalassaemia workup in India should quantify variants rather than simply look for "thalassaemia yes or no".

Points that matter locally:

  • Iron-deficiency anaemia is common and coexists frequently with thalassaemia trait. Assuming that microcytic anaemia equals iron deficiency is a common reason carriers are missed for years.
  • Consanguineous marriage in some communities raises the chance of both partners carrying the same mutation.
  • Premarital and antenatal screening programmes operate in several states and institutions, and the National Health Mission's guidance on haemoglobinopathies supports screening of pregnant women and at-risk groups. Availability differs by state and by hospital.
  • Terminology in reports may use "Hb electrophoresis" and "HPLC" interchangeably even though the methods differ; ask the laboratory which method was used if a result is borderline.
  • Genetic information is personal data under the Digital Personal Data Protection Act, 2023. Ask how your sample and report will be stored and shared.

Энэтхэг дэх зардал ба даатгал

Costs vary considerably across the country and between public and private facilities. Rather than a single price, it helps to understand what drives it:

  • Which test is ordered. A CBC is inexpensive; Hb HPLC costs more; HBB gene sequencing and prenatal molecular diagnosis are substantially more expensive because they need specialised platforms and trained molecular staff.
  • Bundling. A "thalassaemia profile" combining CBC, HPLC and ferritin usually costs less than the same tests ordered separately.
  • Number of people tested. Couple screening and family cascade testing multiply the cost, though testing relatives of a known carrier is often the most efficient use of resources.
  • Referral laboratories. If DNA testing is outsourced, transport and third-party laboratory charges are added.
  • Салбар. Government hospitals, state screening programmes and some NGO-run thalassaemia centres offer screening free or at subsidised rates.

Insurance coverage for outpatient diagnostic tests is limited under many Indian policies; tests done during an admission are more often covered. Screening done purely as a preventive or premarital check is frequently excluded. Check with your insurer or the hospital's TPA desk before testing, and ask whether any state or national programme covers your situation.

"Thalassaemia trait means I am ill."Most people with beta thalassaemia trait have no symptoms or only mild anaemia and a normal lifespan. The main significance is for reproductive planning.
"My haemoglobin is low, so I need more iron."Iron treats iron deficiency. Giving iron to someone with thalassaemia trait and normal iron stores does not correct the anaemia and may contribute to iron accumulation. Iron studies should guide this decision.
"A normal haemoglobin electrophoresis rules out thalassaemia."It does not. Alpha thalassaemia and some silent beta variants are not identified by HPLC and need DNA testing.
"If one of us is a carrier, our child will have thalassaemia major."Thalassaemia major generally requires an abnormal beta-globin gene from both parents. If only one partner is a carrier, children may inherit the trait but are not expected to develop thalassaemia major.
"Thalassaemia can be caught from someone else."Beta thalassaemia is inherited through genes. It is not infectious and is not caused by diet.
"Testing after a transfusion is fine."Recent transfusion makes HPLC-based carrier assessment unreliable. DNA testing or deferring the test is usually needed.
"Thalassaemia only affects certain communities, so I don't need testing."Carrier frequency is higher in some communities, but carriers occur across India. Screening decisions are based on indices, family history and clinical context.

Түгээмэл асуултууд

What is the difference between beta thalassaemia trait and beta thalassaemia major?

Trait means one altered beta-globin gene. It usually causes mild or no anaemia and needs no treatment. Major means both genes are affected, producing severe anaemia from infancy that generally requires regular transfusions and iron-chelation therapy. Intermedia sits between the two, with variable transfusion needs.

Can a CBC alone diagnose beta thalassaemia?

No. A CBC can raise suspicion when MCV and MCH are low with a relatively preserved red cell count, but it cannot separate thalassaemia trait from iron deficiency, and the two often coexist. Confirmation needs haemoglobin HPLC or electrophoresis, iron studies and, in some cases, DNA analysis of the beta-globin gene.

Do I need to fast before a thalassaemia test?

Fasting is not usually required for a CBC or haemoglobin HPLC. If iron studies, lipids or glucose are bundled into the same visit, your laboratory may ask you to fast. Preparation instructions are set by each laboratory, so follow the specific instructions given to you when you book.

Should both partners be tested before pregnancy?

Testing one partner first is common practice; the second partner is tested if the first is a carrier. Risk to a baby generally arises only when both parents carry a beta-globin abnormality. Testing before or very early in pregnancy is preferable, because prenatal diagnostic options depend on gestational age.

Can iron deficiency hide thalassaemia trait on a report?

Yes. Iron deficiency can lower HbA2 enough to bring a true carrier into the normal range, producing a falsely reassuring report. If suspicion is high, for example a strong family history, your doctor may correct iron deficiency first and repeat the HPLC, or move directly to beta-globin gene analysis.

Is genetic testing always needed?

Not always. Many carriers are identified confidently by HPLC combined with indices and iron studies. DNA analysis is added when results are borderline or contradictory, when a variant needs precise identification, when both partners are carriers, or when prenatal diagnosis is being planned.

Does a thalassaemia diagnosis affect my HbA1c or "3 month sugar test"?

It can. Haemoglobin variants and altered red cell turnover may make HbA1c read falsely high or falsely low depending on the laboratory method. Tell your diabetes team about any haemoglobin disorder so they can choose a suitable assay or use alternative ways of assessing glucose control.

Can beta thalassaemia be treated?

Thalassaemia trait usually needs no treatment beyond correcting any coexisting iron or vitamin deficiency. Thalassaemia major and intermedia are managed with regular transfusion, iron chelation and specialist follow-up; haematopoietic stem cell transplantation and newer gene-directed therapies are options in selected patients. Outcomes vary with individual circumstances.

My child's newborn screening was normal. Is that enough?

Newborn screening can identify significant haemoglobin disorders but does not reliably detect beta thalassaemia trait, because foetal haemoglobin is naturally high at birth. If there is a family history or the child later develops unexplained anaemia, repeat testing at an older age is appropriate.
