The adenosine deaminase (ADA) test measures ADA enzyme activity, most often in pleural fluid, to help assess whether a fluid collection is likely to be tuberculous. The ADA test supports, but does not confirm, a tuberculosis diagnosis. Confirmation depends on microbiology, molecular tests such as Xpert MTB/RIF, cytology and sometimes biopsy.
কী টেকওয়েস
- Supportive, not confirmatory: a raised pleural fluid ADA increases the likelihood of tuberculous pleuritis but does not prove it.
- A normal ADA does not exclude tuberculosis: low values have been reported in HIV-related TB and in some early effusions.
- Raised in other conditions too: empyema, parapneumonic effusion, rheumatoid pleuritis and some lymphomas can also raise ADA.
- Sample type matters: ADA behaves differently in pleural fluid, ascitic fluid, cerebrospinal fluid and serum, and thresholds are not interchangeable.
- The invasive part is the fluid sampling: thoracentesis, ascitic tap or lumbar puncture, not the laboratory assay.

- ADA test
- Pleural fluid ADA
- Adenosine deaminase activity / enzyme assay
- Commonly asked for in India as "ADA in pleural fluid", "TB fluid test" or "pleural fluid TB enzyme test"
What the adenosine deaminase test is
Adenosine deaminase is an enzyme of purine metabolism that is present in most cells and is especially active in lymphocytes and monocytes. When T-lymphocytes are activated, as happens in tuberculous inflammation of a serous membrane, ADA activity in the surrounding fluid tends to rise. The ADA test measures that enzyme activity in a fluid sample.
The ADA test is a biochemical marker of lymphocyte activity, not a test that detects যা মাইকোব্যাকটেরিয়াম যক্ষ্মা. No organism, no DNA and no drug-resistance information comes from an ADA result.
A separate and unrelated use of ADA measurement exists in immunology: severely deficient ADA activity in red cells is linked to ADA-deficient severe combined immunodeficiency in infants. That is a specialised paediatric immunology investigation and is not the same as the pleural fluid ADA test discussed here.
Why the adenosine deaminase test is done
- Assessing a lymphocytic exudative pleural effusion where tuberculosis is a realistic possibility and microbiological yield is expected to be low.
- Adding weight to a clinical picture when AFB smear is negative, which is common in tuberculous pleuritis because the fluid contains few organisms.
- Helping decide whether to proceed to pleural biopsy or to treat empirically while awaiting culture, as part of a wider assessment.
- Occasional use in ascitic fluid, pericardial fluid or CSF in suspected tuberculous peritonitis, pericarditis or meningitis, where evidence is weaker and local practice varies.
কাদের এই পরীক্ষাটি করানো উচিত
- Adults with a newly found pleural effusion that is exudative and lymphocyte-predominant, particularly with fever, weight loss, night sweats, cough or pleuritic chest pain.
- People being investigated for suspected tuberculous pericarditis or tuberculous ascites, where the treating team considers ADA useful alongside other fluid tests.
- People in whom pleural fluid AFB smear and cytology are unhelpful and a decision must be made about further invasive testing.
- Who does not need it: people with an obvious transudate from heart failure, cirrhosis or renal disease; people with a confirmed alternative cause; and people with sputum-confirmed pulmonary tuberculosis who already have a microbiological diagnosis. ADA is not a screening test for tuberculosis and has no role in checking a healthy person for "hidden TB".
What the ADA test cannot detect or exclude
- The ADA test does not identify any organism. It cannot distinguish এম। যক্ষ্মা from other causes of lymphocytic inflammation.
- The ADA test cannot detect drug resistance. Rifampicin-resistance information requires Xpert MTB/RIF, line probe assay or culture-based drug susceptibility testing.
- A normal or low ADA does not exclude tuberculosis. Lower values may occur in advanced HIV with poor lymphocyte function, in very early effusions and in some loculated collections. A negative result describes the sample tested, not the whole disease process.
- A raised ADA does not exclude cancer. Malignant effusions are usually associated with lower ADA, but overlap exists; cytology and, where needed, biopsy remain necessary.
- The ADA test says nothing about the lung parenchyma. It does not replace chest imaging or sputum testing.
- Thresholds are not universal. Cut-off values differ by assay method, laboratory and fluid type, and published cut-offs come from populations with differing TB prevalence. A number close to the cut-off should never be read as a yes-or-no answer.
If symptoms continue despite a reassuring ADA result, return to the doctor who requested the test. Persistent breathlessness, fever or weight loss needs re-evaluation regardless of the enzyme value.
সম্পর্কিত কিন্তু ভিন্ন পরীক্ষা
| পরীক্ষা | প্রশ্ন করলে উত্তর পাওয়া যায় | Key difference from ADA |
|---|---|---|
| Pleural fluid ADA | Is lymphocyte activation in this fluid consistent with tuberculous pleuritis? | Biochemical marker; supportive only |
| Xpert MTB/RIF or Ultra on fluid or tissue | Is mycobacterial DNA present, and is rifampicin resistance likely? | Detects the organism's DNA; sensitivity in pleural fluid is limited, so a negative result does not exclude TB |
| AFB smear and mycobacterial culture | Are bacilli visible or growable? | Culture is the reference standard and gives drug susceptibility, but takes weeks and is often negative in paucibacillary fluid |
| Pleural fluid cytology | Are malignant cells present? | Answers the cancer question, not the TB question |
| Pleural biopsy with histology and culture | Is there granulomatous inflammation or malignancy in the pleura? | Highest yield for tuberculous pleuritis; more invasive |
| IGRA (QuantiFERON, T-SPOT) on blood | Has the immune system encountered TB antigens? | Cannot distinguish TB infection from active TB disease; not a test of the effusion |
| Tuberculin skin test (Mantoux) | Same broad question as IGRA | Cannot confirm or exclude active disease; affected by BCG and immunosuppression |
| Red cell ADA activity | Is there inherited ADA deficiency in an infant with immunodeficiency? | Different indication, different sample, different interpretation entirely |
These tests answer different questions. None is universally superior; which comes first depends on the presentation. A patient with productive cough and a cavity on chest X-ray is investigated with sputum tests, not pleural ADA. A patient with an unexplained effusion and no sputum needs fluid analysis.
কিভাবে তৈরী করতে হবে
Preparation relates almost entirely to the sampling procedure, not to the enzyme assay. Instructions differ between hospitals and departments, and your unit's written instructions take precedence over any general guidance.
- ওষুধ: tell the team about blood thinners, antiplatelet drugs, insulin or other glucose-lowering medicines, steroids and immunosuppressants. Do not stop or change any prescribed medicine yourself, speak to the prescriber or the team performing the procedure.
- উপবাস: usually not required for a pleural or ascitic tap. Some units ask for a short fast if sedation is planned. Confirm with your unit.
- Consent and imaging: most units now perform pleural and ascitic taps under ultrasound guidance and will take written consent.
- Clothing and escort: loose clothing helps; an escort is sensible if sedation or a lumbar puncture is planned.
যদি আপনি রক্ত পাতলা করার ওষুধ সেবন করেন অথবা আপনার রক্তক্ষরণজনিত কোনো সমস্যা থাকে
Coagulation results and platelet count are often checked before fluid sampling. Any decision to pause an anticoagulant is made by the prescribing or procedural team, weighing clot risk against bleeding risk.
আপনার যদি ডায়াবেটিস থাকে এবং আপনি গ্লুকোজ কমানোর থেরাপি নিচ্ছেন
If your unit asks you to fast, ask specifically how to handle insulin or sulfonylureas that morning, as unadjusted doses with a missed meal can cause hypoglycaemia.
আপনার যদি হার্ট ফেইলিওর, গুরুতর দীর্ঘস্থায়ী কিডনি রোগ বা তরল গ্রহণের উপর বিধিনিষেধ থাকে
Mention these before the procedure. They affect positioning, how much fluid can safely be drained at one sitting, and the interpretation of the fluid itself, since effusions in these conditions are often transudates in which ADA has little value.
আপনি যদি গর্ভবতী হন
Fluid sampling can be done in pregnancy when clinically necessary, with attention to positioning and to avoiding unnecessary ionising radiation. Tell the team the gestational age so imaging guidance can be planned with ultrasound.
শিশু
Paediatric fluid sampling is performed by clinicians experienced with children, often with sedation or play preparation, and with smaller sample volumes. Adult thresholds should not be transferred to children without paediatric input.
বয়স্ক প্রাপ্তবয়স্কদের
Coexisting heart failure, anticoagulation and frailty influence both procedural risk and interpretation, since more than one cause of effusion may be present at once.
পরীক্ষার সময় কী ঘটে
The laboratory step is simply an enzyme activity assay on the fluid. The part you experience is the sampling.
- Thoracentesis (pleural tap): you sit leaning forward over a support. The site is located, often with ultrasound, cleaned, and numbed with local anaesthetic. A needle or small catheter is passed between the ribs and fluid is withdrawn. Typical total time is around 15-30 minutes; if a larger volume is drained therapeutically it can take longer. Some units perform a chest X-ray afterwards.
- Ascitic tap: you lie on your back; the abdomen is cleaned and numbed and fluid is withdrawn, usually within 15-30 minutes.
- কুমড়া puncture: you lie curled on your side or sit leaning forward. After local anaesthetic, a fine needle collects CSF. Usually 20-30 minutes, with a period of rest afterwards according to unit policy.
- Blood sample for serum ADA: a routine venepuncture taking a few minutes.
Local anaesthetic stings briefly. Pressure or a dull ache during fluid withdrawal is common. Tell the operator immediately if you develop chest pain, severe coughing, dizziness or breathlessness during the procedure.
আপনার রিপোর্ট বোঝা
ADA is reported as enzyme activity per litre, usually U/L or IU/L. The report should state the fluid type, the assay method and the laboratory's own reference or decision value, these differ between laboratories, and the value printed on your report is the one that applies to your sample.
- Higher ADA activity in a lymphocytic exudative effusion shifts probability towards tuberculous pleuritis, especially in a high-TB-burden setting and with a compatible clinical picture.
- Lower ADA activity makes tuberculous pleuritis less likely but does not remove it from consideration.
- Borderline values are common and are interpreted alongside fluid protein, LDH, glucose, cell differential, adenosine-independent microbiology and imaging.
ADA is never interpreted alone and never diagnoses a condition by itself. The doctor who requested the test will read it together with your symptoms, examination, chest imaging, other fluid results and, where relevant, HIV status and treatment history. Results vary depending on individual clinical circumstances.
When ADA results can be misleading
Falsely raised or misleadingly high
- Empyema and complicated parapneumonic effusion: neutrophil-rich infected fluid can show high ADA activity that has nothing to do with tuberculosis.
- Rheumatoid pleuritis and other autoimmune serositis: intense local immune activity raises ADA.
- Lymphoma and some other haematological malignancies involving the pleura or peritoneum.
- Blood-stained (haemorrhagic) samples, since cellular ADA can contribute to the measured activity.
Falsely lowered or misleadingly low
- Advanced HIV with low CD4 count, where the lymphocyte response that drives ADA is blunted.
- Other significant immunosuppression, including high-dose corticosteroids, post-transplant regimens and some biologic therapies.
- Very early sampling, before a lymphocytic response has developed.
- Diluted or partially clotted samples, delayed transport, or storage outside the laboratory's stated conditions, which can reduce measured enzyme activity.
- Loculated collections where the aspirated pocket is not representative.
Assay and threshold variability
Different laboratories use different methods and different cut-offs. Comparing an ADA number from one laboratory with a cut-off quoted by another, or with a value found online, is unreliable. Serial ADA values are not used to monitor response to TB treatment.
ঝুঁকি এবং নিরাপত্তা
The enzyme assay itself carries no risk to you. Risk comes from obtaining the sample, and differs substantially between sampling routes, so they should not be grouped under one safety statement.
Venepuncture for serum ADA
Brief pain, bruising, occasional light-headedness; infection at the site is uncommon.
থোরসেন্টেসিস
Possible pain at the site, bleeding, pneumothorax (sometimes needing a chest drain), infection, and re-expansion pulmonary oedema if a large volume is removed quickly. Ultrasound guidance and experienced operators reduce, but do not remove, these risks.
Ascitic tap
Possible bleeding, leakage of fluid from the puncture site, bowel or bladder injury (uncommon), and infection. People with cirrhosis and coagulopathy need individual assessment.
কটিদেশীয় পাঞ্চ
Post-procedure headache is the most frequent problem. Back pain, transient nerve-root pain, bleeding and infection can occur. Raised intracranial pressure or a space-occupying lesion must be considered before the procedure, which is why imaging is sometimes done first.
Pericardial fluid sampling
Pericardiocentesis carries higher risk than pleural or ascitic sampling, including arrhythmia, cardiac or vessel injury and bleeding. It is performed in a monitored setting by specialists.
লাল পতাকাগুলো
এখন জরুরি অবস্থা

- Coughing up significant blood, or fainting and collapse.
- Rapidly worsening bleeding or swelling at the puncture site.
- Confusion, seizure, new weakness of a limb or facial droop, or severe headache with neck stiffness and fever.
- Chest pain with sweating, low blood pressure or a racing pulse after pericardial sampling.
একই দিন
- Fever above 38癈, spreading redness, warmth or discharge at the puncture site.
- Persistent fluid leakage from the site.
- Headache after lumbar puncture that is severe, worsens on sitting upright and does not settle with rest and fluids.
- Breathlessness that is mild but clearly increasing over hours.
নিয়মিত সাক্ষাৎ
- Mild soreness or bruising settling over a few days.
- Discussing your ADA result and next steps with the requesting doctor.
- Continuing cough, weight loss, night sweats or fatigue despite a reassuring result, this needs reassessment, not reassurance.
বিশেষ পরিস্থিতি
এইচআইভি আক্রান্ত ব্যক্তিরা
Tuberculosis is common and ADA may be lower than expected when CD4 counts are low. Greater weight is given to molecular testing, culture and tissue sampling.
গর্ভাবস্থা
Investigation of a suspected tuberculous effusion is not deferred because of pregnancy. Ultrasound-guided sampling is preferred and radiation exposure is minimised; management is coordinated between chest physicians and obstetrics.
শিশু
Children often cannot produce sputum, so fluid and tissue sampling carry more weight. Paediatric decision thresholds and volumes differ from adults, and results are interpreted by a paediatrician.
Older adults and people with heart failure
Effusions are frequently transudative and related to cardiac or renal disease, where ADA adds little. Two causes may coexist, so fluid characterisation as transudate or exudate comes before ADA interpretation.
People on immunosuppressants or biologics
Immune-modifying treatment can both increase TB risk and blunt the ADA response. Mention every such drug, including recently stopped ones, to the team.
Advanced chronic kidney disease
Uraemic pleuritis and dialysis-related effusions are alternative explanations, and volume shifts affect how much fluid is safe to remove.
The ADA test in the Indian context
India carries a large share of the global tuberculosis burden, and tuberculous pleural effusion is one of the commonest forms of extrapulmonary tuberculosis seen in Indian chest clinics. Because pre-test probability of tuberculosis is high here, a raised pleural ADA carries more practical weight in India than it would in a low-incidence country, and, equally, alternative diagnoses such as malignancy and empyema must still be actively excluded rather than assumed away.
- Terminology you may hear: "ADA in pleural fluid", "fluid TB test", "pleural tap" or "chest ka pani nikalna" for thoracentesis.
- National programme pathway: tuberculosis diagnosis and treatment in India are organised through the National TB Elimination Programme, which prioritises molecular testing such as Xpert MTB/RIF for diagnosis and drug-resistance detection. ADA supports clinical decision-making but is not a programme diagnostic test, and notification of diagnosed TB is mandatory.
- Coexisting conditions that alter the picture: diabetes is common and increases TB risk; anaemia and undernutrition are frequent; HIV co-infection must be considered and tested for with consent.
- Treatment monitoring caution: anti-tubercular therapy can cause liver injury, and herbal, AYUSH or gym-supplement products taken alongside treatment can add to that risk. Tell the treating team about every product you take.
ভারতে খরচ এবং বীমা
Costs quoted below are indicative ranges only and vary widely between laboratories, cities and hospital categories. Confirm the current price with the laboratory or hospital billing desk before the test.
- ADA assay alone: usually a modest additional charge on a fluid analysis panel.
- The larger cost is the procedure and the panel around it: ultrasound guidance, day-care or procedure-room charges, consumables, fluid biochemistry, cytology, AFB smear, molecular testing and culture.
খরচকে প্রভাবিত করে এমন বিষয়সমূহ: whether sampling is done as an outpatient, day-care or inpatient procedure; use of ultrasound or sedation; how many fluid tests are bundled; whether molecular testing and culture are added; NABL-accredited versus non-accredited laboratory; urban versus smaller-town pricing; and repeat sampling if the first aspirate is inadequate.
বীমা: diagnostic tests done as part of an admission or day-care procedure are commonly covered when medically necessary, subject to policy terms, waiting periods and sub-limits. Purely outpatient investigations may not be reimbursed unless your policy includes OPD benefits. Public-sector facilities and National TB Elimination Programme services provide TB diagnostics and treatment at low or no cost. Check specifics with your insurer or the hospital's insurance desk.

সচরাচর জিজ্ঞাস্য
Can the ADA test alone confirm tuberculosis?
No. The ADA test measures enzyme activity linked to lymphocyte activation, not the tuberculosis bacterium. A raised value supports the diagnosis when the clinical picture fits, but confirmation requires molecular testing, culture, cytology or pleural biopsy. Treatment decisions are made on the whole picture, not on one enzyme value.
My pleural fluid ADA is just above the laboratory cut-off. What does that mean?
Borderline values sit in a genuinely uncertain zone. Your doctor will weigh them against fluid protein, LDH, cell differential, imaging, microbiology results and your symptoms. Sometimes the answer is to repeat sampling or proceed to pleural biopsy rather than to accept the number as a verdict either way.
Does the ADA test need fasting?
Fasting is usually not needed for ADA itself. Some units request a short fast when sedation is planned for the sampling procedure. Preparation instructions are unit-specific, so follow the written instructions your hospital gives you, and ask specifically about insulin or other glucose-lowering medicines.
পুরো প্রক্রিয়াটি কত সময় নেয়?
A pleural or ascitic tap generally takes about 15 to 30 minutes including positioning, local anaesthetic and dressing, with a short observation period afterwards. A lumbar puncture usually takes 20 to 30 minutes plus rest time. Reporting time for the ADA assay depends on the laboratory's schedule.
Is the ADA test safe?
The laboratory assay poses no risk to you. Risk relates to obtaining the fluid and differs by route: venepuncture carries minimal risk, thoracentesis carries a risk of pneumothorax and bleeding, and pericardiocentesis carries higher risk and is done in a monitored setting. Results and recovery vary between individuals.
How is ADA different from an IGRA or Mantoux test?
IGRA and the tuberculin skin test assess whether your immune system has encountered tuberculosis antigens at some point; neither distinguishes infection from active disease. Pleural ADA assesses immune activity within a specific fluid collection. They answer different questions and are not substitutes for each other.
Can medicines change my ADA result?
Corticosteroids, immunosuppressants and biologic therapies can blunt the lymphocyte response and lower measured ADA. Never stop or adjust these yourself, tell the team performing the test what you take, and discuss any change only with the doctor who prescribed the medicine.
Can ADA tell whether my TB is drug-resistant?
No. Drug-resistance information comes from molecular tests such as Xpert MTB/RIF or line probe assay, and from culture-based drug susceptibility testing. If drug resistance is a concern, the treating team will arrange these specifically; ADA contributes nothing to that question.
My ADA was normal but I still feel unwell. What should I do?
Go back to the doctor who ordered the test. A normal ADA describes that sample, not your whole illness. Continuing fever, weight loss, night sweats or breathlessness needs reassessment, which may include repeat imaging, further fluid or tissue sampling, or investigation for a non-tuberculous cause.
