What the SGPT test is
Alanine aminotransferase is an enzyme concentrated inside liver cells, with smaller amounts in kidney, heart and skeletal muscle. The enzyme helps transfer amino groups during amino acid metabolism. When liver cells are inflamed, stressed or dying, alanine aminotransferase leaks out and blood levels rise.

Routine SGPT screening in a completely well person with no risk factors is not universally recommended; it is commonly included in Indian health check packages, and an isolated mild abnormality in that setting needs sensible interpretation rather than alarm.
Why the SGPT test is done
- To detect liver cell injury when symptoms or risk factors are present.
- To help characterise a liver problem: the pattern of SGPT versus ALP and GGT helps separate hepatocellular injury from cholestatic (bile flow) problems.
- To monitor over time: trends across repeat tests are more informative than a single reading.
- To monitor drug safety during treatment that can affect the liver.
- To track response in conditions such as chronic viral hepatitis or autoimmune hepatitis, alongside other tests.
What the SGPT test cannot detect or exclude
This is the part most reports leave out, and it matters more than the number itself.
- SGPT cannot tell you the cause. Viral hepatitis, metabolic liver disease, alcohol, medicines, supplements, autoimmune hepatitis, coeliac disease, thyroid disease and muscle injury can all raise SGPT.
- SGPT cannot exclude cirrhosis or advanced fibrosis. Enzyme levels frequently fall towards normal as scarring advances and functioning liver cells are lost. A normal SGPT is not evidence of a healthy liver.
- SGPT cannot grade fat or fibrosis. It says nothing about how much steatosis, inflammation or scarring is present. That requires imaging, elastography, non-invasive scores or occasionally biopsy.
- SGPT cannot detect liver cancer or liver masses. Hepatocellular carcinoma can occur with normal enzymes. Imaging is required.
- SGPT cannot measure liver function. A liver failing to synthesise proteins or clear bilirubin may still show modest enzyme elevation.
- SGPT cannot exclude bile duct obstruction or gallstone disease. ALP, GGT, bilirubin and ultrasound address that question.
- SGPT is not liver-exclusive. Skeletal muscle injury, strenuous exercise and some muscle disorders can raise it without liver disease.
A result reported as normal means the enzyme was not raised in that sample at that time. It does not mean liver disease has been ruled out. If your symptoms continue despite a normal SGPT, go back to your doctor rather than assuming the liver has been cleared.

No single test is universally "first". If the presenting problem is jaundice, bilirubin and a biliary ultrasound often lead. If the problem is an incidental fatty liver with metabolic risk, enzymes plus fibrosis risk assessment lead. If the problem is right upper quadrant pain suggesting gallstones, ultrasound leads.

Understanding your SGPT report
SGPT is reported in units per litre (U/L) or international units per litre (IU/L). Reference ranges vary with assay method, laboratory, sex and age, and some laboratories use lower upper limits than others. Use the range printed on your own report. Do not compare your value against a range from a different laboratory or a website.
Broad principles your doctor applies:
- Mild elevation: commonly metabolic (fatty) liver disease, alcohol, medicines or supplements. Often managed with cause identification, repeat testing and fibrosis risk assessment.
- Marked elevation: raises the possibility of acute viral hepatitis (including hepatitis A and hepatitis E, both common in India), drug-induced liver injury, ischaemic injury or autoimmune hepatitis. These need prompt clinical assessment.
- Pattern matters more than the single figure: SGPT dominant suggests hepatocellular injury; ALP and GGT dominant suggests a cholestatic process.
- Trend matters: a falling SGPT in a patient who is clinically deteriorating is not necessarily good news.
- Nízke SGPT is usually of no clinical concern on its own.
Specific numbers such as 75, 90, 110 or 200 U/L cannot be graded as "mild" or "serious" in isolation. The same value carries very different meaning in a person with a two-day febrile illness and jaundice, in a person on anti-tubercular therapy, and in an asymptomatic person with a fatty liver on sonography. Only the doctor who requested the test, with your history, examination and the rest of the panel, can interpret it. Results vary depending on individual clinical circumstances.

When SGPT results can be misleading
Causes of a falsely or misleadingly raised SGPT

- Haemolysis of the sample during collection or transport can interfere with several analytes.
- Recent alcohol intake or a new supplement may produce a transient rise that resolves on repeat testing.
- Coeliac disease, thyroid disorders and muscle disease can produce persistent mild elevation mistaken for primary liver disease.
Causes of a falsely reassuring or falsely low SGPT
- Established cirrhosis: enzymes often normalise as hepatocyte mass falls, so a normal SGPT may coexist with advanced disease.
- Chronic hepatitis B or C in an inactive phase: normal enzymes despite ongoing infection and possible fibrosis.
- Fluctuating disease: enzymes in chronic hepatitis and MASLD rise and fall, so a single normal value may fall in a quiet window.
- Vitamin B6 (pyridoxine) deficiency and chronic dialysis can lower measured activity.
- Different assay upper limits: some laboratories use higher cut-offs, so a value flagged normal in one laboratory may be flagged high in another.

Mild changes can occur in pregnancy, but raised transaminases are never to be dismissed as normal pregnancy change. Intrahepatic cholestasis of pregnancy, pre-eclampsia with liver involvement, HELLP syndrome, hyperemesis gravidarum and acute viral hepatitis (including hepatitis E, which can be severe in pregnancy in India) all need obstetric assessment.

Paediatric reference ranges differ from adult ranges. Persistent elevation in a child prompts consideration of metabolic, genetic, autoimmune and muscle disorders as well as steatotic liver disease, and should be assessed by a paediatrician or paediatric gastroenterologist.

The SGPT test in the Indian context

- AYUSH, herbal and gym supplements are a recognised and under-reported cause of liver injury in Indian practice. Disclose everything you take, including products sold as "natural" or "liver detox". Claims that a product cures or reverses liver disease also fall foul of the Drugs and Magic Remedies (Objectionable Advertisements) Act, 1954.
- Metabolic liver disease at lower BMI: South Asians develop metabolic dysfunction and steatotic liver disease at lower body mass index and waist thresholds than White European populations, so a "normal-looking" build does not exclude MASLD.
- Haemoglobinopathies and iron-deficiency anaemia are common in India. These do not affect SGPT directly but do affect other tests often ordered in the same visit, such as HbA1c, whose reliability is reduced in thalassaemia trait and other haemoglobin variants.
