    KOLKATA, October 07, 2021: For the first time in Eastern India, doctors at Apollo Multispecialty Hospital gave a new lease of life to an unborn by interstitial laser therapy using Diode laser. In Interstitial Diode Laser Therapy a fine needle is inserted into the mother’s womb to release some laser beams into the sick twin which gets terminated in the process to save its healthier co-twin. The procedure is conducted with the continuous support of ultrasound.Due to more people opting to become parents at a later age and hence the increase of IVF there has been a significant increase of twin babies around the world. Currently, one in fifty pregnancies have twin babies and twin pregnancies are always high-risk pregnancies. Among all twin pregnancies, about 66% are dichorionic (babies form from 2 separate eggs and sperms and have separate compartments with separate blood flow inside the uterus) and 33% are monochorionic (babies form from 1 egg and 1 sperm and have a common compartment with single blood flow inside the uterus).In the case of monochorionic twin pregnancy, there is a high chance of cross (selective) intrauterine growth restriction called Twin-to-twin-transfusion syndrome (TTTS), which is the most important cause of prenatal death of twins. The syndrome typically occurs in the second trimester in about 10-15% of monochorionic twin pregnancies and one of the babies don’t grow as expected and compared to the other. This happens as sharing the same placenta causes blood flow imbalance amongst the twins, where one foetus becomes the so-called donor twin and the other the recipient.If untreated, perinatal mortality is unavoidable as one baby dies due to lack of nutrition and the other due to overnutrition. Secondly, if there is any clot or cardiac dysfunction in the weaker baby due to lack of nourishment, the clot or cardiac issue gets transferred to the healthy baby as well through the common placenta causing the death of the healthy baby within 72 hours of the weaker baby’s death inside the foetus. In this case, the mother was detected with the issue at 14 weeks of pregnancy, when saving both the babies by ensuring proper growth inside the womb was impossible.Interstitial Laser Therapy is the only procedure that can save at least one baby out of the two. In a single treatment procedure, with help of ultrasound in order to see the foetus and placenta, the technique separates the connection of the vessels between the two babies. As a result of this, the weaker donor baby stops receiving nutrition while the healthy baby keeps growing inside the foetus.Dr Jayanta Kr Gupta, Director & HOD Obstetrics & Gynaecology, Apollo Multispecialty Hospitals, Kolkata said, “It’s indeed a tough decision for parents to let go of one of the twins before birth, but this is the only way to save the other baby or else both the babies can’t live. We here at Apollo for the very first time in Eastern India did the procedure of Interstitial Laser Therapy so that we can at least save one of the twins. The treatment was applied in a case of Twin-to-twin-transfusion syndrome (TTTS), which is unique to identical twins, who by nature get nutrition supplied by a single placenta. And we hope to perform on more pregnant women with Twin-to-twin-transfusion syndrome so that they don’t lose their babies due to this condition of intrauterine growth restriction. Developed using a combination of applied AI and clinical expertise on a large sample of retrospective data on health checks and coronary events, the tool delivers a risk score that takes into account all contributing factors including lifestyle attributes such as diet, tobacco & smoking preferences and physical activity as well as psychological stress and anxiety as reflected via rate of respiration, and hypertension. The risk is categorized into High, Moderate and Minimal. The tool also gives insights on the risk contributors that can be modified to improve the score. This enables physicians to give a more holistic advice to their patients with specific insights for lifestyle modification.

    Hyderabad, September 23rd, 2021 : A team of Cardiologists led by Dr Karunakar Rapolu, Consultant Cardiologist, Apollo Hospitals; performed a complex interventional procedure using Impella, IVL and OCT; on an extremely critical 61 year old patient. The patient had 90% blockage due to high calcium deposit in the Left Main Vessel, which was jeopardising the blood supply to already severely weakened heart. A preliminary medical evaluation found him unsuitable for a bypass surgery, due to his past history of Coronary Artery Disease and having undergone an angioplasty three years ago. The Team had to swiftly explore for options to restore blood circulation before his condition deteriorated further and became fatal. The patient Mr Ravinder Reddy, a retired police official, presented with excruciating chest pain and symptoms of severely restricted blood flow to the heart due to calcification of the largest coronary artery, the Left Anterior Descending Artery (LAD). The LAD became narrow progressively since his previous angioplasty and manifested in the form of a massive heart attack. At the Hospital the patient was quickly wheeled into the Cath lab for an angiogram to assess the extent of heart damage. However, he developed dysfunction of the Left ventricular heart chamber, on the table, leading to sparse supply of blood to the vital organs and potential damage of those if not acted instantaneously. This episode further complicated his condition and the team had to resort to administering diuretics to rid excess fluids from the patient’s body and vasodilators to open the blocked vessels.His perilous condition ruled out any scope of performing the risk laden bypass surgery. The team narrowed down on the most apt option considering his delicate condition, of performing a CHIP Intervention (Complex High-risk Indicated Procedure) along with an Impella Heart Pump. CHIP Intervention is a highly critical procedure performed for immediate restoring of blood supply in the circulation system, while Impella replicates the work done by Left Ventricle, helps suck the oxygenated blood from the Left Ventricle and pumps it to the circulatory system. Using the infrared light guidance from OCT (Optical Coherence Tomography), the extent of occlusion was assessed and those high resolution images facilitated clearing of the blockage with precision. Intravascular lithotripsy (IVL) Balloon was used to crack the impregnable calcification of the blockage. IVL deploys acoustic pressure waves to break the calcium deposits. A stenting was done of the Left Main Coronary Artery using DK crush technique for the best outcome.During the course of the procedure the patient suffered 7-8 episodes of “Ventriculo arterial uncoupling” while inflating the balloon to clear occlusion, where the left ventricle ceased pumping the blood. Impella got into action and ensured uninterrupted supply of blood. The patient would have crashed during the procedure if Impella was not availed. The procedure took four and half hours, patient was moved out of ICU after 24 hours of the procedure. He has since recovered and was discharged on the 4th day of admission. He needs to take regular medicines and to follow healthy life style habits like regular exercises and diet management.     More than 250 school children, teachers and gym instructors received training in Basic Life Support and First-Aid today at Apollo Institute of Medical Sciences and Research (AIMSR), Hyderabad. The training in Cardio-Pulmonary Resuscitation (CPR) was done by a team of Life-Support Instructors from Department of Emergency Medicine, Apollo Hyderabad, as a part of the National Assembly on Pediatric Emergency Medicine (NAPEM) conference. NAPEM 2017 is being conducted by the Society for Trauma and Emergency Pediatrics (STEP) in collaboration with the Society for Emergency Medicine India (SEMI), the Indian Academy of Pediatrics (IAP) and Apollo Hospitals. The conference is hosted with the intent of raising the awareness and standard of care for pediatric emergencies in all hospitals.Dr. Imron Subhan, Head of the Apollo Hyderabad Emergency Department, said “In an emergency, bystanders who witness an accident should provide first-aid and quickly get the victim to the hospital. If they hesitate or delay the transportation, then even emergency doctors cannot do much for a patient. Every family should have at least one person trained in Basic Life Support and First-Aid skills”.     Apollo Hospitals performs a complex double organ – Heart & Lung Transplantation!
    Hyderabad, July 2021: A team of doctors led by renowned Cardiothoracic Surgeon and Padmasri awardee Dr. Alla Gopala Krishna Gokhale performed a rare, combined Heart and Lung transplant on 20th July 2021 to save a terminally ill twenty-five-year-old woman, Ms Labdhi Jain. The recipient was unfortunately born with congenital heart disease with a hole in her heart known as a ventricular septal defect and an abnormal connection between her great arteries (Aorto-pulmonary window). Over the years, the congenital defect led to further deterioration in her health condition with increased lung pressures and leakage of heart valves. She was surviving on oxygen support at home for the last two-and-half years. Of late, she was admitted at various hospitals several times for heart failure. The transplant team at Apollo Hospitals, Hyderabad, on evaluation found her to be in an advanced stage of heart failure coupled with increased lung pressures. The team prescribed combined Heart and Lung transplantation to save her life. She was registered under the State Government’s Jeevandan Cadaver Transplantation program and was waiting for a suitable donor. The team faced unique challenges in this case. The recipient at the time of listing for transplant weighed only 30 kgs. Getting a suitable organ which was of appropriate in size for her was complex, as her heart cavity was small. She was helplessly waiting for a suitable organ for a prolonged period of over two and half years, with the hope of finding a small-sized organ diminishing by the day.The cadaver heart and lung obtained from an eighteen-year-old girl, pronounced brain dead due to intracranial bleeding and thrombosis, aptly suited her requirement. Despite the immense sorrow of losing their child, the donor’s family displayed exemplary courage and benevolence and gave consent to harvest their braindead daughter’s organs. This humanitarian gesture on behalf of the grief-stricken loved ones brought joy back in the life of another family.The recipient recovered well after surgery; her weight has improved to 38 kgs.
