    Apollo Multispeciality Hospitals, Kolkata prevented loss of vision in a 6-year-old, Master Rudranjan Sinhababu, who was suffering from severe sinusitis with right sided orbital cellulitis. From the first week of December 2022, the baby, a resident of Jirabad, Bankura had fever along with cold and cough. Master Rudranjan was brought by his worried parents to Dr Shantanu Panja, Senior ENT – Head & Neck Surgeon at Apollo Multispeciality Hospitals, Kolkata on December 10, 2022. Upon receiving the patient, treatment was started immediately and investigations were carried out. CT Scan revealed that the child had extensive sinusitis with right sided orbital cellulitis. An urgent surgery was carried out by Dr Panja and his team on December 12, 2022. As the pressure on the eye was relieved and pus was evacuated the child improved dramatically within 48 hours with resolution of swelling, improvement in vision and complete restoration of colour vision.     Apollo Proton Cancer Centre (APCC)has successfully performed Asia’s first Radical pleurectomy with HITHOC (Hyperthermic intrathoracic chemotherapy) on a 16-year-old adolescent from Kerala. In early 2020, 16-year-old adolescent was detected to have Ewing’s Sarcoma of the left intra thoracic cavity for which the patient underwent neo-adjuvant chemotherapy followed by surgery and radiation with maintenance chemotherapy. Post treatment when the recurrence of the cancer was deducted within 6 months, the patient approached APCC, Chennai for further treatment. The team of clinicians examined the case and conducted tests to chart out the multi-step surgery and treatments for the unique case. The treatment performed by Dr Khader Hussain, Consultant, Surgical Oncologist (Thoracic) & Robotic Surgeon and Dr Abhijit Das, Consultant, Surgical Oncologist (Thoracic) & Robotic Surgeon, consisted of Radical pleurectomy, pericardiectomy, wedge resection of left lung, en-bloc resection of the intrathoracic mass followed by HITHOC (Hyperthermic intrathoracic chemotherapy).     Apollo Hospitals, Navi Mumbai has successfully performed 11 pediatric cardiac interventions on 11 children from Washim district in Maharashtra. The interventions were performed back to back in 11 hours with no morbidity or mortality. These 11 children aged between 4 years and 13 years had congenital heart disease, that if left untreated could lead to serious long-term problems if not treated. Apollo Hospitals, Navi Mumbai also treats patients under the Rashtriya Bal Swasthya Karyakram (RBSK) scheme. Under RBSK scheme, district medical officers screen and identify children with congenital heart diseases and specialist doctors confirm the diagnosis and provide treatment. The treatment provided to the patients are covered under this scheme, thus, helping their families drastically reduce the expenditure incurred on the treatment. Dr Bhushan Chavan, Pediatric Cardiologist, Apollo Hospitals, Navi Mumbai, said, “In September, our team visited Washim Civil Hospital wherein we did echocardiography for 120 children suspected to have congenital heart disease. Of the 120 children, 35 of them had congenital heart disease. Out of 35 children, 40% were planned for treatment through angiography. With permission from the Civil Hospital, the children were brought to Apollo Hospitals, Navi Mumbai for angiography device closure procedure. On the day of the procedure, the doctors started operating by 7:30 am and performed one surgery after another. Out of the 11 cases, five of them had peri-membranous vascular disease which was challenging but yet completed. The children were discharged post-op day three. The entire operation was smooth with no adverse events. These children do not require any other interventions besides follow up at the end of one month and then after 6 months. The remaining children will also be treated at Apollo Hospital, Navi Mumbai. Mr. Santosh Marathe, Regional CEO – Western Region, Apollo Hospitals, said, “At our hospital we have undertaken more than 350 pediatric cardiac cases till date of which 40% were international patients. We have also done 60 cases completely free of cost. All 11 children have been discharged and returned to their homes in Washim with zero complications.     In a rare case, a 29-year-old man was successfully operated for a tumor in his windpipe, at Indraprastha Apollo Hospitals, New Delhi. Initially, the patient had complained of chest pain and progressive swelling in his neck. After suffering for over 2 months he consulted a local doctor who diagnosed him with Tuberculosis, but his condition did not improve with TB medication. As next steps he underwent bronchoscopy that revealed a large mass in his windpipe causing 80% blockage. With his condition deteriorating due to acute breathlessness and fluctuating oxygen levels he was admitted to a local hospital of Faridabad. With no respite in his condition he was referred to Indraprastha Apollo Hospitals, New Delhi where he was admitted under the supervision of Dr Nikhil Modi, Senior Consultant, Respiratory and Critical Care Medicine, Indraprastha Apollo Hospitals, New Delhi. Here, he was immediately put on ventilator to first stabilise his oxygen level. However, when further examined by Dr Suresh Singh Naruka, Senior Consultant, ENT and Head and Neck Surgeon, Indraprastha Apollo Hospitals, New Delhi for his tumor, he was recommended an immediate tumor removal surgery. Dr Suresh Singh Naruka, Senior Consultant, ENT and Head and Neck Surgeon, Indraprastha Apollo Hospitals, New Delhi said, “When this patient was admitted to Apollo he had developed a tumour which was causing 80% blockage of the neck. Hence after stabilizing his oxygen levels an immediate surgery was carried out in order to maintain the airway. This surgery in particular was challenging. Due to the size of the tumor, there was no space for air to pass into the lungs and patient saturation rate was dropping very fast whenever we were doing any instrumentation. The procedure took 3 hours but we were able to successfully remove the tumor. Post-operatively the patient was kept in ICU for further monitoring and as he was successfully weaned off from mechanical ventilation, he was discharged on 9th November, 2022. Post-surgery, the biopsy of wind pipe mass revealed a cancer of lymphoid cells known as Lymphoma. Lymphoma is extremely rare to find in wind pipe as it comprises only 0.2% to 0.3% of all wind pipe tumors. Basis the presence of lymphoma he is undergoing chemotherapy as further management and is keeping well now.” Speaking on the same, Dr Nikhil Modi, Senior Consultant, Respiratory and Critical Care Medicine, Indraprastha Apollo Hospitals, New Delhi said, “Early and correct diagnosis has the best opportunity for a positive heath outcome. Sometimes, with patient getting delayed medical care the condition can become life-threatening. Same was the case with this 29-year-old man from Uttar Pradesh, due to delay in treatment & wrong diagnosis, the tumor increased in size and caused nearly complete blockage of the wind pipe. With his condition deteriorating he was admitted on 29th October, 2022 at Indraprastha Apollo Hospitals. To maintain his airway, he underwent urgent tracheostomy and bronchoscopy followed by tumor removal surgery.” The patient is still visiting the hospital for follow-up and management of Lymphoma through chemotherapy.

Dr Satish Nair, Senior Consultant, ENT & Head and Neck Oncology, Apollo Cancer Centre, Bangalore, along with a team of medical experts performed an endoscopic trans canal excision of facial nerve schwannoma on a 35-year-old patient who was suffering from right-sided facial weakness for the last two years. This is for the first time in the world that the principles of head and neck, lateral skull-base approach, and endoscopic techniques were used together to retain the entire functionality of the ear. The team did an otoendoscopy that revealed a pinkish-pale non-pulsatile mass behind the intact tympanic membrane at the posterosuperior quadrant. Further examination showed that the patient had right-sided Grade V lower motor neuronal facial palsy and the high-resolution computed tomography of Temporal Bone revealed a mass in the facial recess eroding the posterior genu of the facial nerve, which suggests facial nerve schwannoma. The surgery lasted for two hours wherein the team of doctors performed endoscopic trans canal excision of facial nerve schwannoma with cable grafting using the greater auricular nerve. The patient was discharged on the second post-operative day without any complications.     A 64-year-old gentleman from Fiji recently Robot-Assisted Total Arterial Bypass Surgery for Triple Vessel Coronary Artery disease at Indraprastha Apollo Hospitals, Delhi. He had been suffering from chest discomfort for the past 2 years and had an uncontrolled blood sugar levels and impaired kidney function. Additionally, on examination he was also diagnosed with moderate heart dysfunction- his heart was not pumping enough blood, which put him at the risk of heart failure. The patient had been advised an open heart surgery in his native country, however he was skeptical due to the possibilities of multiple complications involved like chest bone being cut, blood loss, kidney failure and post-surgical recovery. The surgery would be challenging because the patient had multiple underlying co-morbidities, hypertension, unstable sugar levels and lower heart function. With his condition deteriorating and taking a toll on his overall health, he was referred to Indraprastha Apollo Hospitals, New Delhi. He was admitted under the supervision of Dr MM Yusuf, Specialist Consultant, Robotics and Minimally Invasive Cardiac Surgery, Dr Mukesh Goel, Senior Consultant, Cardiothoracic Surgery & Dr Varun Bansal, Consultant, Robotics and Minimally Invasive Cardiac Surgery, Indraprastha Apollo Hospitals. As a first step, patient was admitted at the hospital for 5 days prior to the surgery to stabilize his kidney function. Once he was stabilized, the team of doctors conducted the robot-assisted total arterial triple vessel coronary artery bypass surgery. Post-surgery, he was kept for observation in the ICU and then discharged. Currently, he is stable and walking around without support and performing all regular activities independently.     A 60-year-old man, citizen of Nigeria, successfully underwent a high risk and rare surgery for total femur replacement of the whole of thigh bone with knee and partial hip at Indraprastha Apollo Hospitals in New Delhi. Femur replacement surgery is a salvage arthroplasty procedure used as an alternative to lower limb amputation. It is characterized by limb preservation, faster and quality rehabilitation. Total Femur replacement surgeries are done for patients who have undergone cancer related treatment in the past or pathologic fracture of a previously fixed femur which does not get better by other procedures. In this case the patient, Mr. Onah, had been undergoing treatment for recurrent giant cell tumor for the past 12 years. He had undergone two surgeries in South Africa in 2010 following which tumor excision and mega prosthesis was done at Indraprastha Apollo Hospitals in 2013 followed by a revision in 2016. On his recent follow up consultation his reports revealed that he required Total Femur replacement prosthesis. He was admitted at Indraprastha Apollo Hospitals on 17th September and on the same day in a 5 hour long procedure, a total femur replacement was done by Dr Yash Gulati, Senior Consultant Orthopedics, Joint Replacement & Spine, Indraprastha Apollo Hospitals, New Delhi and his team of surgeons. Dr Yash Gulati, Senior Consultant Orthopedics, Joint Replacement & Spine, Indraprastha Apollo Hospitals, New Delhi says that, “Administering a Total Femur Replacement surgery to Mr. Onah, who has undergone tumor excision and mega prosthesis procedures in the past, was challenging. Very few hospitals in the country are equipped to conduct such a complicated procedure. However, the state of the art facility at Indraprastha Apollo Hospitals enabled us to undertake this procedure and save a life. The surgery was high-risk as the patient had multiple surgeries in the same region before and had a big tumor operated three times in the same area along with a total of 5 surgeries earlier. There was a risk of embolism as he was not in a mobile condition for several months. Additionally, there was also a big risk of dislocation due to the lack of proper tissue support. The surgery was the last resort and another alternative option would have been to do an amputation of the whole lower limb. The reason for the ailment was a tumor which Mr. Onah had at the lower end of femur and was operated twice in South Africa and had appeared again. The procedure and post-surgical recovery of Mr Onah was uneventful and after a week the patient was able to move with support. He was discharged on 22nd September. As a treatment modality, he shall be administered regular post-operative physiotherapy for a couple of months.

In this procedure about 90% of the prostate is removed as against conventional methods that take out only 40% to 50% of the enlarged prostate. There is no bleeding or pain caused to the patient and no risk of regrowth of prostate. “It is a minimally invasive, painless with no bleeding and brings down the duration of hospital stay, making recovery quicker. It can also be used to treat enlarged prostate of any size of (as much as 675 grams of prostate which is the largest in the world) and kidney stones (as much as 4.5 to 6.5 cms) and on patients with high risk with hypertension, diabetes, brain stroke and those who are still recovering from complex surgeries and are on blood thinners and anticoagulants. While earlier we had to stop blood thinners on patients for about 5 to 7 days for prostate removal and kidney stone surgeries, Moses laser does not require to stop blood thinners for conducting these surgeries. It doesn’t leave any scar and doesn’t cause any shockwaves or use any puncture methods,” he added. Surgeons require extensive training as Moses 2.0 is technically complicated and demands great technical skills and expertise. Dr Manohar T, the principal investigator on Moses 2.0, with an experience of 16 years on laser, has been studying this unique technology for 2.5 years and have conducted 36 surgeries using Moses 2.0 technology in three months’ time, and has been leading a structured training program at Apollo Hospital, Sheshadripuram. Surgeons from the UK, Sri Lanka, Nepal, Bahrain come here to get hands-on training on Moses 2.0 on advanced simulators that are imported from the USA.     Apollo Hospitals, Navi Mumbai has announced the completion of 53 life-saving pediatric liver transplant surgeries at a special press conference. The Hospital has undertaken total 170 Liver transplants as on date, of which 34 are Cadaver & 12 are International. The significant landmark was achieved in five years and is a testament to the success of Apollo’s Advanced Pediatric Liver Transplant Program in giving a new lease of life to children suffering from end-stage liver disease.     Mrs. Margaret, a 57-year-old lady from Kenya underwent successful treatment for stage IV endometrial cancer (A cancer of inner lining of uterus) with chemotherapy and immunotherapy at Indraprastha Apollo Hospitals, Delhi. The patient had been suffering from endometrial cancer for the past 1 year and was operated in her native country Kenya in November, 2021. However, the disease spread to other organs and the patient developed additional complications involving extensive disease in the abdomen and pelvis, fluid in both lungs, liver metastasis, a non-healing surgical wound in the abdomen and clots in her legs. With her condition deteriorating and disease taking a toll on her overall health she was referred to Indraprastha Apollo Hospitals, New Delhi where she was admitted under the supervision of Dr Shuaib Zaidi, Senior Consultant, Surgical Oncologist at Indraprastha Apollo Hospitals, New Delhi. As a first step, she was given chemotherapy under the able guidance of Dr Ajay Gupta, Senior Consultant, Medical Oncology, Indraprastha Apollo Hospitals, New Delhi. Speaking on the same Dr Shuaib Zaidi, Senior Consultant, Surgical Oncologist at Indraprastha Apollo Hospitals, New Delhi said, “Endometrial cancer is the cancer of the inner lining of uterus. Often it is diagnosed at an early stage due to frequent abnormal vaginal bleeding but in few cases it can reach to Stage IV without being diagnosed. In most cases of stage IV endometrial cancer, the cancer can spread too far to be removed with surgery and so needs chemotherapy and immunotherapy for treatment. This was the case with Mrs. Margaret, from Kenya who presented with extensive peritoneal deposits in abdomen and pelvis, chest nodes, fluid in both lungs, extensive disease in liver, a non-healing surgical wound and clots in both legs after surgery of Stage IV Endometrial cancer. On admission, the patient was very weak and fatigued due to multiple health complications. She was admitted in the hospital on 20th January, 2022 for initial management of disease but with her condition deteriorating due to large volume of disease she was advised chemotherapy with immunotherapy. So far, in world literature such an excellent response to low dose immunotherapy and chemotherapy has not been described in endometrial cancer.” Talking about the care Dr Ajay Gupta, Senior Consultant, Medical Oncology, Indraprastha Apollo Hospitals, New Delhi said, “On admission at Indraprastha Apollo Hospitals, Mrs. Margaret was very weak and was unable to walk. She needed chemotherapy to combat the widespread disease process. Considering her poor general condition, we started her on weekly chemotherapy. Post two months of therapy, she showed reasonable physical improvement and the disease process had stabilised. The PET scan was also showing stable disease. However, there was a danger that this high risk disease might progress when the chemotherapy was stopped. To circumvent this problem, we utilized a novel approach using low dose immunotherapy, which was very affordable and this was added to her chemotherapy regimen. Following three cycles of Immunotherapy and six cycles of chemotherapy a PET scan was done to assess the response to treatment. It was seen that the blood clots, along with the extensive metastatic disease in the lungs, abdomen, pelvis and liver had disappeared suggesting a complete/ near complete response of this deadly disease to the given therapy. She has recovered well and is now back to her country with recommendation to continue immunotherapy there. She is now without any symptoms and leading a normal, healthy active life.” Both Dr Ajay Gupta and Dr Shuaib Zaidi are elated with these results and feel that it is indeed heartening that the latest advances in immunotherapy in combination with conventional chemotherapy are having such a significant, positive impact in patient’s life at a cost they can afford.
