What the Apo B test is
Cholesterol does not travel freely in blood. Cholesterol is carried inside lipoprotein particles. Each atherogenic particle, LDL, VLDL, IDL, chylomicron remnants and lipoprotein(a), carries exactly one molecule of apolipoprotein B-100 (or apoB-48 in intestinal particles). Measuring Apo B therefore estimates how many such particles are circulating.
This matters because particles, not cholesterol molecules, enter and lodge in the artery wall. Two people can have an identical LDL cholesterol value while one carries far more, smaller, cholesterol-poor particles. That person generally carries more risk, and the Apo B test is what reveals the difference.
Apo B is measured on an automated analyser using an antibody-based method standardised against an international reference material. Values are reported in mg/dL in most Indian laboratories, and in g/L in some (1 g/L = 100 mg/dL).
Why the Apo B test is done
- Refining cardiovascular risk when a standard lipid profile gives an incomplete or contradictory picture.
- Assessing residual risk on treatment: some people reach their LDL cholesterol target but still have a high particle number.
- Evaluating combined dyslipidaemia: raised triglycerides with low HDL, typical of insulin resistance and metabolic syndrome.

Who should consider getting an Apo B test

Who usually does not need an Apo B test
A young adult with no risk factors, a normal lipid profile and no family history usually gains little from the Apo B test. Routine population screening with Apo B is not standard practice. The decision belongs with the doctor who knows your history.
What the Apo B test cannot detect or exclude
This is the most commonly misunderstood part of Apo B testing.
- Apo B does not visualise arteries. The Apo B test says nothing about whether plaque exists, where it is, or how narrowed a vessel is. Imaging such as coronary CT angiography, a coronary calcium score or invasive angiography answers those questions.
- A normal Apo B does not exclude coronary artery disease. Plaque can be present with a perfectly acceptable Apo B value, particularly where smoking, hypertension, diabetes, chronic kidney disease, inflammation or family history dominate. If you have chest discomfort, breathlessness on exertion or reduced exercise tolerance despite a normal Apo B, return to your doctor.
- Apo B does not exclude a future cardiac event. Risk is probabilistic, not binary.
- Apo B does not measure lipoprotein(a) separately. Lp(a) particles contain apoB and contribute to the total, but a high Lp(a) can be missed if only Apo B is measured. Lp(a) requires its own test, usually once in a lifetime.
- Apo B does not separate LDL from VLDL or remnant particles. A raised Apo B does not tell you which particle type predominates.
- Apo B does not assess heart function, rhythm or valves. An echocardiogram (echo test) and ECG answer those questions.
- Apo B does not diagnose diabetes, thyroid disease or liver disease, though all three can influence the result.
- Apo B does not by itself confirm familial hypercholesterolaemia. Diagnosis uses clinical scoring criteria and, where available, genetic testing.
A negative or normal result describes what was measured on that day. It is not the same as an abnormality being ruled out.

None of these tests is universally superior. How to prepare for the Apo B test
Preparation requirements are operational and vary between laboratories and hospitals. The instruction given by your own collection centre and requesting doctor takes precedence over any general guidance.
- Jejum: Apo B measurement is generally reliable in the non-fasting state, and many international lipid guidelines now accept non-fasting sampling. Some Indian laboratories still request 9-12 hours of fasting, particularly when a full lipid profile is drawn at the same time. Confirm with the unit before your visit.
- Medicamentos: Do not start, stop or change any prescribed medicine, including statins, ezetimibe, fibrates, thyroid medication or steroids, to prepare for this test. Tell the requesting doctor what you take, and ask the prescriber if any change is needed.
- suplementos: Mention fish oil, niacin, red yeast rice, AYUSH preparations and gym or bodybuilding supplements, several of which affect lipids or liver function.

- Population reference intervals differ from treatment goals. Your laboratory report will usually print a reference interval derived from a reference population. That interval is not the same thing as your personal target. Many people sit "within the reference range" and are still advised to lower Apo B further because of their risk category.
- Units matter. Check whether your report is in mg/dL or g/L before comparing two reports.
- Discordance is informative. If Apo B is proportionately higher than LDL cholesterol, particle number exceeds what the cholesterol value suggests, and risk is generally better tracked by Apo B.
Ask the doctor who requested the test what Apo B target applies to you. Do not set a target from an internet figure, and do not use a laboratory reference interval as a treatment goal. Results require interpretation in clinical context by the requesting doctor.
What a raised Apo B does and does not mean
A raised Apo B means more atherogenic particles were circulating on the day of the sample. It is a risk marker. It does not mean you have blocked arteries, does not predict when an event might occur, and by itself does not mandate any specific medicine. Results vary depending on individual clinical circumstances.
When Apo B results can be misleading
Situations that can falsely raise or transiently increase Apo B

Risks and safety of the Apo B blood test

Risks are low but not zero. Bruising, brief pain and a small haematoma are the usual issues. Feeling faint occurs in a minority of people, tell the phlebotomist beforehand if you have fainted at blood draws previously. Infection at the puncture site is uncommon.
