The C4 complement blood test measures the amount of complement protein C4 in blood serum. Doctors use the C4 test mainly to help assess suspected lupus, immune-complex kidney disease and hereditary angioedema, and to follow disease activity over time. The C4 test alone does not diagnose any condition and must be read with symptoms and other tests.
ძირითადი მიღება
- The C4 test measures one protein in the complement cascade, usually alongside C3 and sometimes CH50.
- Low C4 may reflect complement consumption (as in active lupus or some nephritis) or an inherited deficiency; it is not specific to one disease.
- A normal C4 result does not exclude lupus, vasculitis or hereditary angioedema.
- C4 is an acute-phase protein, so infection or inflammation can push the level up and mask consumption.
- Interpretation belongs to the doctor who ordered the C4 test, using your clinical picture.

აგრეთვე ცნობილი, როგორც
- C4 complement test, complement C4 level, serum C4
- C4 blood test, "complement test" (often ordered as "C3 C4")
- Commonly referred to in India as the "C3 C4 test" when both are ordered together
What the C4 complement blood test is
Complement is a group of blood proteins that work in a chain reaction as part of the immune system. Complement helps label bacteria for destruction, clears immune complexes (clumps of antibody bound to antigen) and triggers inflammation. C4 sits in the classical and lectin pathways of that chain.
When the classical pathway is heavily activated, C4 is used up faster than the liver replaces it, so the measured level falls. The C4 test therefore gives an indirect signal about immune activity. Some people also have genetically low C4 because of variation in C4 gene copy number, or a true inherited deficiency, without any active disease.
Why the C4 test is done
- Suspected systemic lupus erythematosus (SLE): low C3 and C4 support, but do not prove, active immune-complex disease.
- Monitoring lupus activity: serial C4 and C3 values, alongside anti-dsDNA and urine findings, can help track flares and response to treatment.
- Glomerulonephritis and unexplained proteinuria or haematuria: the C3/C4 pattern helps sort out post-infectious, membranoproliferative, lupus and C3 glomerulopathy-type problems.
- Suspected hereditary angioedema (HAE): recurrent swelling of lips, face, hands, genitals, gut or airway without itching or hives, especially with a family history. C4 is a screening step; C1 inhibitor level and function are needed to characterise HAE.
- Recurrent or unusual infections with encapsulated organisms, where a complement deficiency is being considered.
- Cryoglobulinaemia and some vasculitis syndromes, where C4 is often disproportionately low.
ვინ უნდა ჩააბაროს ეს ტესტი
- People with features suggesting lupus, persistent joint pain with rash, photosensitivity, mouth ulcers, unexplained anaemia, pleuritic chest pain, abnormal urine tests, when a clinician requests complement studies.
- People already diagnosed with lupus or lupus nephritis who need activity monitoring.
- People with repeated episodes of non-itchy, non-hive swelling lasting more than a day, unresponsive to antihistamines, or with unexplained recurrent abdominal pain attacks.
- Anyone with a first-degree relative diagnosed with hereditary angioedema or an inherited complement deficiency.
- People with newly found kidney inflammation on urine testing or biopsy.
The C4 test is not a general health-check or screening test. It has little value in someone without relevant symptoms or a specific clinical question, and an isolated abnormal value in that setting often causes unnecessary worry and further testing.
What the C4 test cannot detect or exclude
- It cannot diagnose lupus. Lupus diagnosis rests on clinical features plus antibodies such as ANA and anti-dsDNA. Some people with active lupus have normal C4.
- A normal C4 does not exclude hereditary angioedema. C4 can be normal between attacks in some patients, and is normal in HAE with normal C1 inhibitor. C1 inhibitor quantity and function testing is required.
- It does not distinguish consumption from inherited deficiency. A persistently low C4 with normal C3 and no disease activity may simply reflect gene copy number variation.
- It does not identify which organ is affected. Low C4 says nothing about kidney, brain or joint involvement; that needs urine tests, creatinine, imaging or biopsy.
- It does not measure complement function. A normal C4 concentration can coexist with a dysfunctional pathway; CH50 assesses overall classical pathway activity.
- It does not exclude infection, malignancy or drug reaction as the cause of your symptoms.
- A negative or normal result is a description of what was measured at that moment, not proof that disease is absent. If symptoms continue despite a normal C4, go back to your doctor rather than assuming the problem has been ruled out.
დაკავშირებული, მაგრამ განსხვავებული გამოკვლევები
| ტესტი | კითხვა მას პასუხობს | ძირითადი განსხვავება C4-დან |
|---|---|---|
| C3 complement | Is the alternative/common pathway being consumed? | Ordered with C4; the C3:C4 pattern is more informative than either alone |
| CH50 (კომპლენტის საერთო აქტივობა) | Is the whole classical pathway functioning? | Functional assay; can be low when individual components are normal. Sample handling is strict |
| C1 inhibitor level and function | Is hereditary or acquired angioedema present? | Required to confirm HAE; C4 is only a screening pointer |
| C1q antibodies / C1q level | Acquired angioedema, hypocomplementaemic urticarial vasculitis | Different analyte, different indications |
| ANA, anti-dsDNA, ENA panel | არსებობს თუ არა ლუპუსთან ასოცირებული აუტოანტისხეულები? | Antibody tests support diagnosis; C4 reflects activity, not diagnosis |
| CRP, ESR | ანთებაა? | Non-specific inflammation markers; CRP is often normal in active lupus while C4 falls |
| Urine routine, ACR, creatinine | Is the kidney involved? | Organ assessment, which C4 cannot provide |
Which of these comes first depends on the presentation. For recurrent non-itchy swelling, C4 with C1 inhibitor testing is the relevant route. For suspected lupus, antibody testing usually leads and C4 adds activity information. For abnormal urine findings, kidney function and urine studies lead. No single test is universally superior.
როგორ მოვამზადოთ
Preparation rules are operational and set by each laboratory, so the instructions given by your collection centre take precedence over anything written here.
- სამარხვო: usually not needed for C4 alone. If lipids or glucose are bundled in the same request, fasting may apply.
- მედიკამენტები და დანამატები: list everything, including steroids, immunosuppressants, biologics, AYUSH preparations and gym supplements. Do not start, stop or alter any prescribed medicine to prepare for this test, speak to the prescriber first.
- ბოლოდროინდელი დაავადება: tell the laboratory and your doctor about any infection, fever or vaccination in the previous few weeks, as these can shift complement levels.
- Angioedema attacks: mention whether you are currently having an attack or are between attacks; timing affects how results are read.
- ჰიდრატაცია: normal fluid intake makes venepuncture easier. If you have heart failure, advanced chronic kidney disease or a prescribed fluid restriction, do not increase your fluid intake for a blood test, keep to your prescribed limit.
თუ ორსულად ხართ
Complement proteins commonly rise during pregnancy, so a "normal" C4 may hide consumption. Interpretation in pregnancy, especially where lupus or pre-eclampsia is being considered, needs an obstetrician working with a rheumatologist or physician.
ბავშვები
The C4 test can be done in children, including infants, using small-volume paediatric tubes. Reference intervals differ with age, so paediatric results should be read against age-appropriate values by a paediatrician. Topical anaesthetic and a parent's presence help.
Older adults and people on immunosuppressants
Fragile veins may need a smaller needle and a more experienced phlebotomist. Immunosuppressive and steroid therapy can influence complement levels and disease activity, so results are read in the context of current treatment.
Bleeding disorders and blood thinners
Tell the phlebotomist if you take warfarin, a direct oral anticoagulant, dual antiplatelet therapy, or have haemophilia or another bleeding disorder. Pressure is applied longer after the draw. Do not stop anticoagulation for a routine blood test.
რა ხდება ტესტის დროს
A phlebotomist cleans a spot on the inner elbow, applies a tourniquet and draws a few millilitres of blood into a plain or serum-separator tube. The needle is in the vein for well under a minute. Including registration, checks and post-draw pressure, most people are in and out within 5-10 minutes. Serum is separated in the laboratory; complement proteins are sensitive to delays and warm storage, so prompt processing matters.
You can eat, drink and return to normal activity straight away. Keep the plaster on for a few hours and avoid heavy lifting with that arm for the rest of the day if you bruise easily. No recovery period is needed.
თქვენი ანგარიშის გაგება
C4 is reported as a concentration, most often in mg/dL or g/L, with a reference interval printed beside the result. That interval is specific to the assay and analyser used, so compare your value only with the range on your own report, and ideally track serial results from the same laboratory. Reference intervals for children differ from adults.
The pattern matters more than a single number:
- Low C4 with low C3: suggests classical pathway consumption, seen in active lupus, lupus nephritis, some post-infectious glomerulonephritis and serum sickness.
- Low C4 with normal C3: raises the possibility of hereditary or acquired angioedema, cryoglobulinaemia, or inherited/partial C4 deficiency.
- Normal C3 with normal C4: does not exclude autoimmune disease; it makes active immune-complex consumption less likely at that moment.
- Raised C4: usually non-specific. C4 behaves as an acute-phase protein and can rise with infection, inflammation, trauma or malignancy. A high C4 on its own rarely changes management.
Rising C4 and C3 back towards the reference interval in someone treated for lupus often accompanies improvement, while a fall can precede a flare. These are trends, not verdicts. Results vary depending on individual clinical circumstances, and the doctor who requested the test is the person who should interpret it.
როდესაც C4 შედეგები შეიძლება შეცდომაში შემყვანი იყოს
Falsely raised or inappropriately normal C4
- თანმხლები ინფექცია ან ანთება: the acute-phase rise can lift a consuming level back into the reference interval, masking disease activity.
- ორსულობა: physiological rise in complement proteins.
- Oestrogen-containing medication can increase complement protein levels.
- Testing between angioedema attacks may show a level closer to normal than during an attack.
- Gene copy number variation means some people naturally sit high and could drop substantially yet still read "normal".
Falsely lowered C4
- Delayed separation or warm transport of the sample: complement is heat- and time-sensitive; in vitro activation lowers measured levels.
- განმეორებითი გაყინვა-დათბობის ციკლები of stored serum.
- Inherited partial C4 deficiency or low gene copy number: a constitutionally low value with no active disease.
- Advanced liver disease or severe malnutrition: reduced hepatic synthesis, unrelated to autoimmunity.
- Heavy protein loss in nephrotic syndrome or protein-losing enteropathy.
- Haemolysed samples may not be suitable and may need a repeat draw.
Because of these factors, a single unexpected C4 value is often repeated on a fresh, properly handled sample before conclusions are drawn.
რისკები და უსაფრთხოება
Routine venepuncture for C4
Risks are small but not zero: brief pain, bruising, a small haematoma, light-headedness or fainting, and rarely local infection or nerve irritation. Bruising is more likely on anticoagulants or antiplatelets. Tell staff beforehand if you have fainted during blood tests previously so you can be drawn lying down.
Paediatric and difficult-access draws
Small children and people with fragile or scarred veins may need more than one attempt, which carries a slightly higher chance of bruising and distress. A capillary or paediatric-volume sample may be used where the laboratory permits it.
ტესტირების შემდგომი რისკები
The bigger risk with complement testing is misinterpretation, an isolated low C4 in a well person can lead to anxiety and avoidable further investigation. Ordering the C4 test for a clear clinical question limits this.

განსაკუთრებული სიტუაციები
ორსულობა
Complement levels normally rise in pregnancy, and falling C3/C4 in a pregnant woman with lupus can signal a flare that overlaps clinically with pre-eclampsia. Joint obstetric and rheumatology or nephrology input is needed. Hereditary angioedema attacks can become more frequent in pregnancy and around delivery, and require a specialist plan made in advance.
ბავშვები
Persistently low C4 in a child with recurrent bacterial infections or an early-onset lupus-like illness deserves a paediatric immunology opinion. Reference intervals vary with age, and neonatal complement levels are physiologically lower than adult levels.
ქრონიკული თირკმლის დაავადება და ნეფროზული სინდრომი
Protein loss through the kidney can lower measured C4 independently of immune activity. In lupus nephritis, complement is interpreted alongside urine protein, creatinine and, where done, biopsy findings.
ღვიძლის ქრონიკული დაავადება
The liver makes complement proteins, so cirrhosis or advanced liver disease can lower C3 and C4 without autoimmune consumption.
People on immunosuppressive therapy
Steroids, mycophenolate, hydroxychloroquine, azathioprine and biologics change disease activity and can influence complement. Never adjust these medicines based on a C4 result on your own; the treating specialist decides.
C4 კომპლემენტის ტესტი ინდურ კონტექსტში
- Availability: C4 and C3 are widely available in medium and large diagnostic laboratories in Indian cities. CH50 and C1 inhibitor functional assays are offered by fewer laboratories and samples are often referred out, which affects both cost and turnaround.
- Sample logistics matter locally: complement is temperature- and time-sensitive. In warm climates and where samples travel long distances from collection camps to a central laboratory, inadequate cold-chain handling can lower measured C4 falsely. Prefer a centre that processes the sample on site or has a documented transport protocol.
- Infection overlap: tuberculosis, enteric fever, dengue, malaria, hepatitis B and hepatitis C are common in India and can each produce fever, joint pain, rash, cytopenias or kidney findings that mimic lupus. Complement results in such settings must be read together with an infection workup, since infection can both mimic and mask autoimmune activity.
- Hepatitis C and cryoglobulinaemia: a markedly low C4 with a relatively preserved C3 should prompt consideration of cryoglobulinaemia, for which hepatitis C is a recognised association.
- ACE inhibitor-related angioedema: ACE inhibitors are widely prescribed in India and can cause angioedema with normal C4. This is a different problem from hereditary angioedema and is managed by reviewing the prescription with the treating doctor, not by stopping the tablet without advice.
- მონაცემთა დაცვა: laboratory reports and any online booking or report-download tool handle personal health data, which is governed by the Digital Personal Data Protection Act, 2023. Check what consent you are giving when sharing reports over messaging apps.
- Prescription practice: complement testing is a specialist-guided investigation. Under NMC professional conduct expectations, tests should be ordered for a defined clinical question rather than bundled into general packages.
ღირებულება და დაზღვევა ინდოეთში
Costs for a serum C4 test in India are modest compared with functional complement assays, and vary widely between centres. Rather than quoting a figure, ask for the current rate when booking, and confirm whether it is for C4 alone or a C3 and C4 combination.
ფაქტორები, რომლებიც ცვლის ფასს:
- Whether C4 is ordered alone or as part of a C3/C4 or autoimmune panel.
- Assay method used (for example nephelometry or turbidimetry) and analyser platform.
- Whether the test is performed in-house or referred to a reference laboratory.
- City, tier of city, and whether the centre is a hospital laboratory, standalone chain or government facility.
- Accreditation status of the laboratory (NABL-accredited laboratories may price differently).
- Home collection charges, urgent processing, and repeat testing on a fresh sample.
- Add-on tests commonly ordered together, ANA, anti-dsDNA, CH50, C1 inhibitor level and function, urine studies.
On insurance: outpatient diagnostic tests are often not covered by standard Indian indemnity health policies unless linked to an admission or covered by an OPD rider or corporate plan. When testing is part of an inpatient episode for lupus nephritis or angioedema, it is usually covered as part of the hospitalisation. Confirm with your insurer or the hospital's insurance desk before assuming cover.

ხშირად დასმული შეკითხვები
რამდენ ხანს გრძელდება მთელი პროცესი?
The blood draw itself takes under a minute, and the full visit including registration and post-draw pressure is usually 5-10 minutes. Laboratory processing time depends on whether the C4 assay is run in-house or referred out, so ask your collection centre for its current reporting schedule.
Is the C4 test the same as the C3 test?
No. C3 and C4 are different complement proteins, and they are usually measured together because the combination is more informative. Low C3 with low C4 suggests classical pathway consumption, while low C4 with normal C3 points towards angioedema, cryoglobulinaemia or inherited C4 deficiency.
Do I need the CH50 test as well?
Not routinely. CH50 measures whether the whole classical pathway works, which is useful when a complement component deficiency is suspected in someone with recurrent serious bacterial infections. CH50 needs careful sample handling. Your treating doctor decides whether CH50 adds value in your situation.
Can medicines change my C4 result?
Yes. Steroids and immunosuppressants alter disease activity and can influence complement levels, and oestrogen-containing medication can raise complement proteins. List everything you take, including AYUSH and over-the-counter supplements. Do not stop or change any prescribed medicine before testing without asking the prescriber.
Can the C4 test be done during pregnancy?
Yes, a venous blood sample is safe in pregnancy. Interpretation is the difficulty, because complement levels normally rise in pregnancy and a falling value in someone with lupus may signal a flare that overlaps with pre-eclampsia. Obstetric and specialist input is needed together.
What if my C4 is low but I feel completely well?
A persistently low C4 in a well person with no other abnormality can reflect inherited low C4 gene copy number or a handling problem with the sample. A repeat on a fresh, promptly processed sample, along with C3 and a clinical review, is the usual next step rather than immediate extensive testing.
რა სიხშირით უნდა განმეორდეს C4 ტესტი?
There is no fixed schedule. In active lupus or lupus nephritis, complement is often checked at each review during treatment changes, then less frequently when stable. In hereditary angioedema, repeat testing is usually for confirmation rather than monitoring. The treating specialist sets the interval.
Is the C4 test suitable for children?
Yes, using paediatric-volume tubes and, where available, topical anaesthetic. Age-specific reference intervals apply, so a paediatrician should interpret the result. Repeated attempts may be needed in small children, which can cause bruising and distress but no lasting harm.
My C4 is normal but my symptoms continue. What now?
A normal C4 describes what was measured on that sample; it does not prove that immune or inflammatory disease is absent. Return to the doctor who ordered the test. Further evaluation may include antibody testing, urine studies, C1 inhibitor testing, imaging or a specialist referral.
