What the AFP test is

Alpha-fetoprotein is a protein made in large amounts by the fetal liver and yolk sac. Levels fall sharply after birth and remain low through adult life. Certain tumours, particularly hepatocellular carcinoma and some germ cell tumours, can start producing AFP again, which is why the AFP test is used as a tumour marker.

In pregnancy, AFP crosses into the mother's blood. The maternal serum AFP concentration changes week by week, so results in pregnancy are reported against gestational age, usually as a multiple of the median (MoM) rather than as a plain number.

The AFP test is a laboratory immunoassay. Different laboratories use different analysers and assay kits, so values are not always directly comparable between labs. For monitoring over time, using the same laboratory is preferable.

Why the AFP test is done

  • Liver cancer surveillance: in people with cirrhosis or chronic hepatitis B or C, AFP may be used alongside six-monthly abdominal ultrasound.
  • Assessment of a liver lesion: AFP adds information when a mass is found on imaging, but does not replace imaging or biopsy.

What the AFP test cannot detect or exclude

  • The AFP test cannot diagnose cancer. It measures one protein. Diagnosis needs imaging, and often tissue or cytology.
  • A normal AFP does not exclude hepatocellular carcinoma. A substantial proportion of liver cancers, including many small early tumours, do not raise AFP. This is why ultrasound remains the backbone of surveillance.
  • A normal AFP does not exclude a germ cell tumour. Pure seminomas and pure dysgerminomas typically do not produce AFP. Choriocarcinoma raises hCG rather than AFP.
  • AFP does not tell you where a tumour is, how large it is, or whether it has spread. Only imaging can address that.
  • AFP is not a test of liver function. It says nothing about bilirubin, albumin, clotting or fibrosis stage.
  • In pregnancy, AFP screening cannot diagnose anything. It cannot exclude Down syndrome, cannot exclude a neural tube defect (closed defects usually do not raise AFP), and does not assess most other birth differences.
  • A negative AFP result means AFP was not raised in that sample: not that disease has been excluded. If symptoms continue despite a normal result, return to your doctor for review rather than assuming all is well.

The AFP test and prenatal AFP screening are not the same examination in practice, even though both measure the same protein. Reference frameworks, reporting units and follow-up pathways differ entirely.

What happens during the AFP test

  • Registration and consent for the requested test.

Points that help make sense of the number:

  • Mildly raised AFP is common in chronic liver disease without cancer, especially during a hepatitis flare or active liver regeneration.
  • A stable mildly raised value over time is interpreted differently from a value that is climbing steadily. Serial trend is often more informative than a single reading.
  • A markedly raised AFP in someone with cirrhosis and a liver mass is clinically significant and triggers prompt specialist imaging, but even then, diagnosis rests on imaging characteristics, and sometimes biopsy.
  • In germ cell tumours, AFP is interpreted with beta-hCG and LDH, and post-treatment decline is assessed against the expected half-life of the marker.
  • 妊娠中, the report gives a risk estimate (for example 1 in 1,200) rather than a yes or no. "Screen positive" means further assessment is advised, not that a condition is present. Most women with a screen-positive result go on to have a normal detailed scan.

When AFP results can be misleading

Causes of a falsely raised or non-cancer-raised AFP

  • Acute or chronic hepatitis, including hepatitis B, C and E, and hepatitis flares.

  • AFP-non-secreting tumours: many hepatocellular carcinomas, pure seminoma, pure dysgerminoma.
  • Small tumour burden: early lesions frequently produce AFP levels within the reference range.
  • Recent treatment: surgery, chemotherapy or ablation lowers AFP; a normal value soon after treatment does not confirm complete clearance.
  • Very high concentrations can occasionally cause assay "hook effect" and a spuriously low reading; the laboratory rechecks on dilution if suspected.
  • In pregnancy: overestimated gestational age, higher maternal weight (dilutional), and closed neural tube defects, which typically do not leak AFP.

The blood draw itself

Venepuncture is a low-risk procedure. Possible effects include brief pain, bruising, light-headedness or fainting, and rarely a small haematoma or local infection. Risks of the downstream pathway

Any ultrasound performed in pregnancy in India is governed by the PC-PNDT Act, 1994; fetal sex determination is prohibited, and registered facilities require the prescribed consent documentation. Genetic counselling should accompany any screen-positive result so that you can make an informed choice about further testing.
