The estradiol (E2) test is a blood test that measures estradiol, the main form of oestrogen made by the ovaries, testes and fat tissue. Doctors use the estradiol test to assess ovarian function, puberty, fertility treatment cycles, menopausal symptoms and some hormone therapies. Estradiol results must be read alongside the menstrual cycle day, age and symptoms.
ძირითადი მიღება
- The estradiol test measures one hormone at one moment; estradiol changes hour to hour and day to day across the menstrual cycle.
- A single estradiol value rarely diagnoses anything on its own, it is usually interpreted with FSH, LH, prolactin, AMH, TSH or ultrasound findings.
- Timing matters. Many ovarian-function assessments use a sample on day 2 or day 3 of the cycle, but your treating doctor decides the day.
- Reference intervals differ between laboratories and between assay methods. Read your result against the range printed on your own report.
- A normal estradiol level does not exclude infertility, endometriosis, PCOS or a pituitary disorder.

- E2 test
- Serum estradiol / oestradiol
- 17-beta estradiol
- Oestrogen blood test (loosely used; oestrogen is a group of hormones, not one)
- Commonly asked for in Indian labs as "E2 hormone test" or part of a "hormone profile" or "infertility profile"
What the estradiol test is
Estradiol is the most biologically active oestrogen during the reproductive years. In women it comes mainly from the developing ovarian follicles. In men it is produced in smaller amounts by the testes and by conversion of testosterone in fat and other tissues. The placenta produces large amounts in pregnancy.
The estradiol test measures the concentration circulating in blood at the moment the sample is taken. Most laboratories use an automated immunoassay. Some specialised laboratories use liquid chromatography-mass spectrometry (LC-MS/MS), which performs better at the very low concentrations seen in children, men and postmenopausal women.

An estradiol test is usually requested, rather than self-selected. It is commonly appropriate for:
- Women with absent or irregular periods: to help distinguish ovarian failure from hypothalamic or pituitary causes, alongside FSH, LH and prolactin.
- Couples being evaluated for infertility: usually as part of a cycle-day-specific panel.
- Women in an IVF or ovulation-induction cycle: serial estradiol measurements track follicular response and help reduce the risk of ovarian hyperstimulation.
- Children with early or delayed puberty: interpreted by a paediatric endocrinologist, ideally with a sensitive assay.
- Women with menopausal symptoms where the picture is unclear, particularly under 45 years, where premature ovarian insufficiency is being considered.
- People on gender-affirming hormone therapy, for monitoring as directed by the prescribing clinician.
- Men with gynaecomastia, low libido or infertility, usually alongside testosterone, LH, FSH and prolactin.
Estradiol is generally არ useful as a routine "wellness panel" add-on in a woman with regular cycles and no symptoms, and it is not a screening test for cancer.
Why the estradiol test is done
- Assess whether the ovaries are producing oestrogen.
- Help work out the cause of amenorrhoea or irregular cycles.
- Monitor follicular growth during assisted reproduction.
- Support the assessment of precocious or delayed puberty.
- Investigate gynaecomastia or feminising features in men.
- Monitor certain hormone therapies, where the prescriber wants a level.
- Occasionally, to investigate a suspected oestrogen-secreting tumour.
What the estradiol test cannot detect or exclude
This is the part most reports leave out.
- It does not diagnose PCOS. Estradiol is often unremarkable in PCOS. Diagnosis rests on clinical features, androgen levels and ovarian morphology, applied together.
- It does not measure ovarian reserve on its own. Anti-M黮lerian hormone (AMH) and antral follicle count on ultrasound are the usual reserve markers. A day-3 estradiol is used mainly to check whether an early-cycle FSH value can be trusted.
- It does not confirm or exclude ovulation. A mid-luteal progesterone is the relevant test.
- It does not diagnose or monitor breast or endometrial cancer. Estradiol is not a tumour marker. It has a narrow role in specific treatment settings only, decided by an oncologist.
- It does not exclude endometriosis, fibroids or ovarian cysts. Those are imaging or surgical diagnoses.
- It is not a pregnancy test. Beta-hCG is.
- A normal estradiol does not exclude a pituitary or hypothalamic disorder, thyroid disease or hyperprolactinaemia.
Results that appear "not identified" or normal describe what the assay measured on that sample, not what exists in the body. If symptoms persist despite a normal estradiol result, return to your doctor.
დაკავშირებული, მაგრამ განსხვავებული გამოკვლევები
| ტესტი | კითხვა მას პასუხობს | How it differs from estradiol |
|---|---|---|
| v | Is the pituitary driving the ovaries harder than usual? | Rises as ovarian function declines; usually paired with estradiol, not replaced by it |
| LH | Is there an ovulatory surge, or an abnormal LH:FSH pattern? | Short-lived surge; timing is critical |
| AMH | Roughly how large is the remaining follicle pool? | Fairly stable across the cycle; estradiol is not |
| შუა ლუთეალური პროგესტერონი | Did ovulation occur this cycle? | Estradiol cannot answer this |
| Estrone (E1) | Dominant oestrogen after menopause | Different molecule; rarely needed clinically |
| Estriol (E3) / unconjugated estriol | Part of some pregnancy screening panels | Not interchangeable with estradiol |
| Prolactin, TSH | Other hormonal causes of menstrual disturbance | Often abnormal when estradiol is low |
| მენჯის ულტრაბგერითი | Structure: follicles, endometrium, cysts, fibroids | Anatomy, not hormone level |
No single test is universally "first". Which comes first depends on the presenting problem: prolactin and TSH often lead in amenorrhoea with galactorrhoea; AMH and antral follicle count lead in fertility planning; estradiol with FSH leads when premature ovarian insufficiency is suspected.
როგორ მოვამზადოთ
Preparation is set by the laboratory and by the doctor who ordered the test. Where your unit's written instruction differs from anything below, follow your unit.
- ციკლის დრო: if you menstruate, ask which cycle day. Day 1 is the first day of full flow. Turning up on the wrong day can make the result uninterpretable.
- სამარხვო: usually not needed for estradiol alone. If other tests are booked in the same draw, fasting may be required for those.
- მედიკამენტები და დანამატები: bring a full list, including oral contraceptives, hormone therapy, fertility drugs, steroids, clomifene, letrozole, anti-epileptics and any AYUSH, herbal or gym supplements. Do not start, stop or change any prescribed medicine to prepare for this test, speak to the prescriber first.
- ბიოტინი: high-dose biotin in hair, skin and nail supplements can interfere with several immunoassays. Tell the laboratory if you take it.
- Დღის მონაკვეთი: some units prefer morning samples so results are comparable across visits.
თუ ორსულად ხართ
Estradiol rises substantially and progressively in pregnancy. Routine estradiol testing is not part of standard antenatal care, and the result should only be interpreted by your obstetrician in context.
ბავშვები და მოზარდები
Very low concentrations need a sensitive assay. A standard adult immunoassay may not be reliable at paediatric levels. Testing for puberty problems should be arranged through a paediatrician or paediatric endocrinologist.
People using topical or vaginal oestrogen
Gel, patch or cream residue on the skin or fingers can contaminate a blood sample and give a spuriously high reading. Mention the route and timing of your last dose.
Older adults and postmenopausal women
Levels are low, and many routine assays lose precision at the bottom of their range. Small numerical differences at this level often mean very little.
რა ხდება ტესტის დროს
A phlebotomist cleans the skin, applies a tourniquet and draws a small blood sample, usually from the inner elbow. The needle is in place for well under a minute. Most people are in and out within 5-10 minutes. You can eat and return to normal activity straight away unless another test required fasting. Press firmly on the site for a couple of minutes to reduce bruising.
During IVF monitoring, the same draw may be repeated every one to three days alongside ultrasound follicle tracking.
Understanding your estradiol report
Your report will show a number, the unit, and a reference interval. Compare your value only with the interval printed on your own report, estradiol reference intervals are method-specific and vary between laboratories, and published ranges from other sources may not apply to your assay.
ექიმის მიერ განხილული ზოგადი ნიმუშები:
- Low estradiol with high FSH: suggests the ovaries are not responding, seen in menopause and, at younger ages, premature ovarian insufficiency.
- Low estradiol with low or normal FSH and LH: points towards a hypothalamic or pituitary cause, such as significant weight loss, intense exercise, chronic illness, stress, hyperprolactinaemia or a pituitary lesion.
- Estradiol within the expected range: indicates ovarian oestrogen production at that moment. It does not confirm ovulation or fertility.
- High estradiol: expected mid-cycle, in pregnancy and during ovarian stimulation. Outside those settings it may prompt a search for an ovarian cyst, an oestrogen-producing tumour, liver disease or, in men, causes of increased conversion of testosterone to oestrogen.
- მამაკაცებში: estradiol is interpreted with testosterone, LH and FSH, and clinical findings such as gynaecomastia. An isolated number is of limited value.
No result should be self-interpreted. Results vary depending on individual clinical circumstances, and only the clinician who requested the test can place the number in context.
When estradiol results can be misleading
Causes of a falsely raised result
- Skin contamination from oestrogen gel, patch or cream, including transfer from a partner.
- Cross-reactivity in immunoassays with other steroids or with fertility drug metabolites.
- Heterophile antibodies or, in some assays, high-dose biotin.
- Haemolysed or improperly stored samples.
- Sampling near the mid-cycle peak when a baseline was intended.
Causes of a falsely lowered or misleadingly low result
- Sampling early in the follicular phase or soon after menstruation, when levels are naturally at their lowest.
- Some biotin-sensitive assays lower rather than raise the reading, depending on assay design.
- Poor analytical sensitivity of routine immunoassays at very low concentrations, as in children, men and postmenopausal women.
- Recent use of combined oral contraceptives, which suppress endogenous estradiol; most also contain ethinylestradiol, which many assays do not measure.
დაბნეულობის სხვა წყაროები
- Switching laboratories or assay platforms between tests, the numbers are not always directly comparable.
- Reporting in pmol/L in one report and pg/mL in another.
- Acute illness, significant weight change or heavy training shifting results temporarily.
რისკები და უსაფრთხოება
სისხლის აღება
Risks are small but real: brief pain, bruising, light-headedness or fainting, and rarely a small haematoma or local infection. People on blood thinners or with bleeding disorders should tell the phlebotomist and press the site for longer.
Risks of over-testing and over-interpretation
An unnecessary estradiol test can generate a borderline number that leads to repeat testing, imaging and anxiety without changing management. This is the more common harm in practice.

განსაკუთრებული სიტუაციები
ორსულობა
Estradiol rises throughout pregnancy and peaks near term. Routine measurement adds little to standard antenatal care. Any prenatal testing in India is governed by the PC-PNDT Act 1994 where imaging is involved; sex determination is prohibited by law.
ბავშვები
Interpretation requires age- and Tanner-stage-appropriate paediatric reference data and a sensitive assay. Adult ranges do not apply.
Women on hormone therapy
Estradiol monitoring is sometimes used with transdermal or implanted oestrogen, less so with oral preparations. The prescriber decides whether monitoring is useful and at what interval. Never adjust a dose based on a laboratory value alone.
Women on combined oral contraceptives
These suppress natural estradiol production. A low result during contraceptive use usually reflects the pill, not ovarian failure.
Chronic kidney disease, liver disease and heart failure
Chronic liver disease alters oestrogen metabolism and binding proteins, and can genuinely raise estradiol in men. Chronic illness of any kind can suppress the hypothalamic-pituitary-ovarian axis. Results in these groups need specialist interpretation.
Thyroid disease and hyperprolactinaemia
Both commonly disturb menstrual cycles and are usually checked alongside estradiol rather than after it.
The estradiol test in the Indian context
- ტერმინოლოგია: patients and labs often say "E2 hormone test", "female hormone test" or include it in an "infertility profile". Ask exactly which hormones a bundled profile contains, since contents differ between laboratories.
- Anaemia and nutrition: iron-deficiency anaemia and undernutrition are common and can accompany hypothalamic menstrual disturbance. A low estradiol in a young woman with weight loss or heavy periods should prompt a broader assessment, not an isolated hormone repeat.
- მეტაბოლური დაავადება დაბალი BMI-ის დროს: South Asians develop insulin resistance and PCOS-related features at lower body mass index than many other populations, so a "normal" build does not rule out a metabolic contribution.
- დამატებები: high-dose biotin in over-the-counter hair and skin supplements is widely used in India and is a recognised interference in immunoassays. Declare it.
- Self-ordered panels: direct-to-consumer hormone packages are widely marketed. Without a cycle day and a clinical question, an estradiol number is difficult to act on. Advertising that promises a cure for hormonal problems falls foul of the Drugs and Magic Remedies (Objectionable Advertisements) Act 1954.
- Assay variation: laboratories across India use different platforms. For serial monitoring, such as an IVF cycle, stay with one laboratory wherever possible.

Pricing for a standalone serum estradiol test varies widely between cities, laboratory chains, government facilities and hospital-based laboratories. Factors that influence what you pay:
- Whether the estradiol test is standalone or bundled into a hormone or infertility profile.
- Assay method, LC-MS/MS testing generally costs more than routine immunoassay.
- Urgency, with same-day or stat processing typically priced higher.
- სახლში ნიმუშების შეგროვების საფასური.
- Number of repeat samples, which can be substantial during ovarian stimulation monitoring.
- City and whether the laboratory is NABL-accredited.
On insurance: outpatient diagnostic tests are frequently not reimbursed unless the policy includes an OPD benefit. Fertility investigation and treatment are excluded under many Indian health insurance policies, though coverage has been broadening. Check your policy wording and pre-authorisation requirements before booking a series of tests.
მითები და ფაქტები
| მითი | ის ფაქტი, |
|---|---|
| A single estradiol test shows whether you can conceive | Fertility assessment needs cycle timing, AMH or antral follicle count, tubal assessment and a semen analysis for the partner. Estradiol alone answers none of this. |
| High estradiol means cancer | High estradiol is expected mid-cycle, in pregnancy and during fertility stimulation. Oestrogen-secreting tumours are rare, and estradiol is not a cancer screening test. |
| You must fast 8-12 hours before an estradiol test | Fasting is generally not required for estradiol. The earlier version of this page stated otherwise; that was incorrect. Fasting may still apply if other tests share the same draw. |
| Low estradiol always means menopause | Low estradiol also occurs with contraceptive use, hypothalamic suppression, hyperprolactinaemia, pituitary disease and chronic illness. |
| PCOD and PCOS are different diseases | They refer to the same condition. PCOS is the correct clinical term. |
| Any laboratory's reference range can be used | Reference intervals are assay-specific. Use the range printed on your own report. |
ხშირად დასმული შეკითხვები
On which day of my cycle should the estradiol test be done?
It depends on the question being asked. Assessment of ovarian function is often done on day 2 or day 3 of the cycle, counting the first day of full flow as day 1. Fertility cycle monitoring uses repeated samples at different points. Confirm the day with the doctor who ordered the test.
Do I need to fast before an estradiol test?
Fasting is generally not needed for estradiol alone. If your requisition includes lipids, glucose or insulin, fasting may be required for those. People with diabetes on glucose-lowering medicines should ask their prescriber before fasting, as skipping meals can cause hypoglycaemia.
რამდენ ხანს გრძელდება მთელი პროცესი?
The blood draw itself takes under a minute, and the full visit is usually 5-10 minutes including registration and pressure on the puncture site. Laboratory processing and reporting times differ between units, so ask the collection centre when your report will be ready.
Can the estradiol test tell me if I have PCOS?
No. PCOS is diagnosed from a combination of irregular ovulation, clinical or biochemical androgen excess and polycystic ovarian morphology on ultrasound, after excluding other causes. Estradiol is frequently unremarkable in PCOS and is not part of the diagnostic criteria.
Does estradiol measure ovarian reserve?
Not directly. AMH and antral follicle count are the usual reserve markers. An early-cycle estradiol is mainly used to check whether a same-day FSH result can be trusted, because a raised early estradiol can suppress FSH and mask reduced reserve.
Can men have this test?
Yes. Estradiol is measured in men being assessed for gynaecomastia, infertility, reduced libido or suspected hormone-producing tumours, usually alongside testosterone, LH, FSH and prolactin. Because male levels are low, a sensitive assay gives more reliable results than a standard immunoassay.
Will birth control pills affect my result?
Yes. Combined oral contraceptives suppress your own estradiol production, and most contain ethinylestradiol, which many assays do not detect. A low reading while on the pill usually reflects the medicine. Do not stop a prescribed contraceptive to get a test, discuss timing with your doctor.
My estradiol is slightly outside the printed range. Is that a problem?
Often not. Estradiol swings widely across the cycle and between days, and assays lose precision at low concentrations. A value just outside the interval, without symptoms, frequently needs nothing more than review in context. Your doctor decides whether repeating or extending testing is worthwhile.
Can lifestyle change my estradiol level?
Marked weight loss, very heavy training, poor sleep, significant stress and acute illness can suppress the hypothalamic-pituitary-ovarian axis and lower estradiol. Chronic liver disease can raise it. These effects are real but are not reasons to self-treat with supplements; discuss persistent symptoms with a doctor.
