The ferritin test is a blood test that measures serum ferritin, the protein that stores iron. A low ferritin level is a reliable sign of depleted iron stores. A normal or high ferritin level does not rule out iron deficiency, because ferritin also rises with infection, inflammation, liver disease and obesity.
Հիմնական ճարպերը
- Low ferritin is meaningful: a clearly low serum ferritin indicates low body iron stores in almost all situations.
- Normal ferritin is not reassurance: ferritin is an acute-phase reactant and can be falsely raised by inflammation, infection, liver injury or alcohol use.
- High ferritin usually is not haemochromatosis: in Indian practice, inflammation, fatty liver (MASLD) and alcohol-related liver disease are far more common explanations.
- Ferritin is one part of iron studies: transferrin saturation, serum iron, TIBC, CBC and CRP are often needed alongside it.
- The blood draw takes a few minutes; interpretation must be done by the doctor who requested the test, in your clinical context.

- Iron store test / "iron storage test"
- Commonly asked for in India as part of "anaemia profile" or "iron studies"
What the ferritin test is
Ferritin is the protein shell inside which the body stores iron, mainly in the liver, spleen and bone marrow. A small amount leaks into the bloodstream, and the ferritin test measures that circulating fraction. In a healthy person without inflammation, serum ferritin tracks total body iron stores reasonably well.
Ferritin is measured by immunoassay. Different analysers and different laboratories use different methods, so the printed reference interval on your report is the one that applies to your result. Results from two laboratories are not always directly comparable.
Why the ferritin test is done
- To confirm iron deficiency in someone with anaemia, fatigue, hair fall, breathlessness on exertion or pica.
- To detect iron deficiency before anaemia develops: iron stores fall first, haemoglobin falls later.
- To monitor iron therapy or response after treating a source of blood loss.
- To investigate a persistently raised ferritin found incidentally, which may point to inflammation, liver disease, alcohol use or, less often, iron overload.
- To assess iron status before certain treatments, such as intravenous iron, erythropoiesis-stimulating agents in CKD, or before some surgeries.
- In transfusion-dependent thalassaemia and other chronic transfusion settings, to help track iron accumulation over time.
Ո՞վ պետք է անցնի այս թեստը
- Adults with unexplained tiredness, breathlessness, dizziness, brittle nails, hair thinning or restless legs.
- Anyone with a low haemoglobin or low MCV on a CBC.
- Women with heavy or prolonged menstrual bleeding.
- Pregnant women and women planning pregnancy, where iron deficiency is common in India.
- Adolescents, particularly girls, and people on strictly vegetarian or restricted diets.
- People with gastrointestinal blood loss, piles, peptic ulcer disease, coeliac disease, or after bariatric surgery.
- People with chronic kidney disease, heart failure or inflammatory bowel disease, where iron deficiency is frequent and easily missed.
- Regular blood donors.
- Anyone with a family history of haemochromatosis, or with unexplained liver enzyme elevation, joint pain or diabetes appearing together.
Whether the ferritin test is appropriate for you is a decision for your treating doctor.
What the ferritin test cannot detect or exclude
- It cannot exclude iron deficiency. Because ferritin rises in infection, inflammation, malignancy, liver disease and after surgery, a "normal" ferritin can coexist with genuinely empty iron stores. In inflammatory states, doctors often use a higher decision threshold and add transferrin saturation and CRP.
- It cannot diagnose the cause of anaemia. Anaemia in India is frequently multifactorial, iron deficiency, vitamin B12 or folate deficiency, thalassaemia trait, haemoglobinopathies, chronic disease and worm infestation can overlap.
- It cannot diagnose haemochromatosis. A raised ferritin alone is not iron overload. Transferrin saturation, repeat testing, liver assessment and sometimes genetic testing are needed.
- It cannot quantify iron in the liver or heart. That requires specialised MRI techniques such as T2* or R2 imaging, used mainly in transfusion-dependent patients.
- It cannot find the source of bleeding. A low ferritin tells you iron is being lost or not absorbed; endoscopy, colonoscopy or gynaecological assessment may be needed to find out why.
- It cannot distinguish thalassaemia trait from iron deficiency on its own, both can cause small red cells. Haemoglobin electrophoresis or HPLC is required.
A normal ferritin result describes what was measured on that sample, not what does or does not exist in your body. If your symptoms continue despite a normal result, go back to your doctor.
Առնչվող, բայց տարբեր քննություններ
| փորձարկում | Հարց, որ պատասխանում է | Key difference from ferritin |
|---|---|---|
| Serum iron | How much iron is circulating right now | Fluctuates through the day and after meals or iron tablets; poor measure of stores |
| TIBC / transferrin | How much iron-carrying capacity is available | Typically rises in iron deficiency, falls in inflammation, opposite direction to ferritin |
| Տրանսֆերինի հագեցվածություն | What proportion of carrier protein is loaded with iron | More useful than ferritin for suspected iron overload; low in deficiency |
| Ընդհանուր արյան ստուգում՝ կարմիր բջիջների ինդեքսներով | Is there anaemia, and are the red cells small | Detects the consequence of iron deficiency, not the iron store itself |
| CRP | Is inflammation present | Helps decide whether a ferritin value can be taken at face value |
| Soluble transferrin receptor | Is the marrow iron-starved | Less affected by inflammation; not available everywhere and costlier |
| Hb electrophoresis / HPLC | Is there thalassaemia trait or a haemoglobin variant | Explains small red cells with normal iron stores |
| Liver MRI (T2*/R2) | How much iron is actually in the liver or heart | Imaging-based quantification; ferritin is only an indirect surrogate |
These tests answer different questions. Which one comes first depends on the clinical problem: for suspected iron deficiency anaemia, CBC and ferritin usually lead; for suspected iron overload, transferrin saturation with ferritin is the standard starting pair; for microcytosis with normal ferritin in an Indian patient, haemoglobin studies are often the next step.
Ինչպես պատրաստել
Preparation rules are set by each laboratory, and the instructions given by your collection centre take precedence over anything written here.
- Պահք: ferritin alone does not usually need fasting. If the ferritin is bundled with a lipid profile or fasting glucose, the laboratory may ask for an overnight fast.
- Iron tablets and syrups: do not stop any prescribed supplement on your own. Tell the requesting doctor what you are taking and when you last took it; many units prefer the sample before the morning dose, or a short gap after starting oral iron.
- Recent intravenous iron or blood transfusion: inform the laboratory and your doctor, as both can raise ferritin substantially for weeks.
- Վերջերս ախտահարված հիվանդություն. fever, infection, injury or surgery in the preceding weeks can lift ferritin. Mention it.
- Ալկոհոլը ` regular alcohol intake raises ferritin. Report your usual intake honestly rather than changing it only for the test.
- Ժամկետները: ferritin varies less through the day than serum iron, but some units still prefer morning samples when full iron studies are ordered.
Եթե հղի եք
Ferritin normally declines through pregnancy because of plasma expansion and fetal iron demand. Interpretation thresholds differ from those for non-pregnant adults. Prolonged fasting is generally avoided in pregnancy, ask the unit for a non-fasting slot if a fast has been advised.
Եթե դուք ունեք շաքարային դիաբետ, որը գլյուկոզի իջեցման բուժման կարիք ունի
If a fast has been requested for bundled tests, discuss timing with your prescriber first. Fasting while on insulin or sulfonylureas carries a risk of hypoglycaemia. Do not skip or shift your medicine without advice from the doctor who prescribed it.
Եթե դուք ունեք ՔՀՀ, սրտային անբավարարություն կամ հեղուկի սահմանափակում
Do not load up on water before the draw. Standard "drink plenty of water" advice is not safe for everyone. Follow the fluid limits your treating team has set.
Երեխաներ և տարեց մեծահասակներ
Paediatric samples are small-volume and reference intervals differ by age. Long fasting is not appropriate for young children. In older adults, a low ferritin should prompt a search for gastrointestinal blood loss rather than being treated as a dietary issue by default.
Ի՞նչ է կատարվում թեստի ընթացքում և որքա՞ն ժամանակ է տևում այն
- A phlebotomist cleans a spot on your arm and applies a tourniquet.
- A needle is inserted into a vein and a small tube of blood is drawn.
- The needle is removed and pressure applied for a minute or two.
- Time at the counter: the draw itself takes under two minutes; expect about 5-10 minutes overall, longer if it is a busy morning or if several tests are being collected.
- Laboratory processing and reporting time varies by unit: ask your collection centre for their reporting schedule.
Understanding your ferritin report
Read your ferritin value against the reference interval printed on your own report, in the units your laboratory uses (usually ng/mL, numerically the same as 礸/L). Reference intervals differ by sex, age, assay and laboratory, so quoting a range from a website can mislead. Your doctor will also consider whether inflammation is present before deciding what the number means.
Ցածր ֆերիտին
A clearly low ferritin points to depleted iron stores. The next question is always ինչու, menstrual blood loss, gastrointestinal bleeding, poor absorption such as in coeliac disease or after gastric surgery, chronic hookworm infestation, or inadequate dietary iron. In men and in postmenopausal women, unexplained iron deficiency warrants investigation of the gastrointestinal tract.
Normal ferritin
A ferritin within the reference interval is compatible with adequate iron stores, but does not prove it if CRP is raised or a chronic inflammatory condition is present. Symptoms that persist should be re-discussed with your doctor.
High ferritin
Most raised ferritin results are not caused by iron overload. Common explanations include infection, any inflammatory or autoimmune condition, metabolic dysfunction-associated steatotic liver disease (MASLD, formerly called NAFLD), MetALD and alcohol-related liver disease, malignancy, recent transfusion or intravenous iron, and chronic kidney disease. Genuine iron overload, hereditary haemochromatosis or transfusional iron loading, is generally suggested by a raised ferritin together with a high transferrin saturation. Very high ferritin values can also occur in severe inflammatory syndromes and require urgent clinical assessment rather than reassurance.
Results vary depending on individual clinical circumstances, and no ferritin value should be interpreted in isolation.
When ferritin results can be misleading
Falsely raised ferritin (masking iron deficiency)
- Acute or chronic infection, including tuberculosis, and any febrile illness
- Inflammatory and autoimmune disease such as rheumatoid arthritis
- Liver disease, including MASLD, MetALD, hepatitis and alcohol-related liver injury, ferritin leaks from damaged liver cells
- Recent surgery, trauma or heavy exercise
- Չարորակություն
- Քրոնիկ երիկամային հիվանդություն և դիալիզ
- Recent blood transfusion or intravenous iron infusion
- Excess alcohol intake
- Hyperthyroidism and some metabolic conditions
Falsely lowered or misleadingly low-normal ferritin
- Hypothyroidism and vitamin C deficiency can be associated with lower values
- Very rarely, assay interference or extreme haemodilution, such as in late pregnancy, can pull the value down
Սխալի այլ աղբյուրներ
- Recently started oral iron can lift ferritin before stores are genuinely replete, giving a falsely reassuring picture.
- Changing laboratories mid-treatment makes trends hard to read, because assays are not interchangeable.
- A haemolysed or delayed sample can affect results and may need repeating.
Ռիսկերը և անվտանգությունը
Ստանդարտ երակային արյան նմուշառում
Complications are uncommon and usually minor: bruising, brief soreness, a small haematoma, or light-headedness. Keeping firm pressure on the site for a minute or two reduces bruising.
Եթե դուք ընդունում եք արյան նոսրացնողներ կամ ունեք արյունահոսության խանգարում
Tell the phlebotomist before the draw so that pressure can be held longer. Do not stop anticoagulants or antiplatelet medicines for a blood test unless your prescriber has specifically told you to.
Երեխաներ և դժվար երակներ ունեցող մարդիկ
More than one attempt is sometimes needed. Asking for an experienced paediatric phlebotomist is reasonable.

հղիություն
Iron deficiency is common in Indian pregnancies and ferritin is the usual marker used to assess stores. Values fall physiologically as pregnancy advances, so pregnancy-specific thresholds apply. Treatment decisions, including oral versus intravenous iron, belong with your obstetrician.
Երեխաներ եւ դեռահասներ
Iron deficiency affects learning, attention and growth. Ferritin in children is interpreted against age-specific intervals. In a child with a small-cell picture and normal ferritin, thalassaemia trait should be considered.
Հին մեծահասակները
Unexplained iron deficiency in an older adult should not be assumed to be dietary. Gastrointestinal evaluation is usually recommended.
Քրոնիկ երիկամային հիվանդություն
Ferritin is routinely raised in CKD by inflammation, so nephrologists use different decision thresholds and rely on transferrin saturation alongside ferritin when deciding on iron therapy.
Սրտի կանգ
Iron deficiency is common in heart failure even without anaemia, and specific criteria combining ferritin and transferrin saturation are used by cardiologists.
Thalassaemia and transfusion-dependent patients
Serial ferritin is used to track iron loading, but it is an imperfect surrogate; liver and cardiac MRI give a more direct measurement where available.
Ferritin testing in the Indian context
- Anaemia is widespread in India, particularly among women of reproductive age, adolescent girls and young children, as documented in the National Family Health Survey (NFHS-5, 2019-21). Iron deficiency is a major but not the only contributor.
- Thalassaemia and haemoglobin E traits are common in several Indian populations. Small red cells with a normal or high ferritin should prompt haemoglobin HPLC rather than empirical iron.
- Infection load matters. Tuberculosis, chronic parasitic infestation and recurrent febrile illness all raise ferritin and can mask iron deficiency.
- Liver disease is under-recognised. MASLD occurs at lower BMI in South Asians and is a frequent reason for a mildly raised ferritin. Anti-tubercular therapy, herbal and AYUSH preparations and gym supplements can cause drug-induced liver injury with secondary ferritin elevation.
- Self-medication with iron tablets is common. Taking iron without a diagnosis can delay identification of a bleeding source and, in someone with iron loading, may cause harm.
- Բառապաշար: patients often ask for the "iron test" or "anaemia profile", clarify with the laboratory whether serum ferritin is actually included in the package.
Արժեքը և ապահովագրությունը Հնդկաստանում
Serum ferritin is a widely available and relatively inexpensive test in India, though prices differ considerably between cities, laboratory chains, government facilities and hospital-based labs. A bundled "iron studies" or "anaemia profile" panel usually costs more than ferritin alone.

| A normal ferritin means my iron is fine | Inflammation, infection and liver disease can push ferritin into the normal range despite empty stores |
| High ferritin means too much iron | Most raised ferritin results reflect inflammation or liver disease, not iron overload |
| Tiredness always means low iron | Thyroid disease, B12 deficiency, sleep disorders, depression and chronic illness cause similar symptoms |

Հաճախակի տրվող հարցեր
Can I have iron deficiency with a normal ferritin?
Yes. Ferritin is an acute-phase protein and rises with infection, inflammation, liver disease, kidney disease and obesity. In these situations a normal ferritin can hide depleted iron stores. Your doctor may add CRP and transferrin saturation, or use a higher ferritin cut-off, to interpret the result properly.
Should I stop my iron tablets before the ferritin test?
Do not stop any prescribed medicine on your own. Tell the requesting doctor what you take and when. Recent oral iron, and especially intravenous iron or a transfusion, can raise ferritin and make stores look better than they are, so timing is decided by your treating doctor.
Does a high ferritin mean I have haemochromatosis?
Usually not. Hereditary haemochromatosis is uncommon and is suggested by a raised ferritin together with a persistently high transferrin saturation, often with a family history. Far more often a raised ferritin reflects inflammation, alcohol intake, fatty liver disease, infection or chronic kidney disease.
How soon after starting iron treatment should ferritin be repeated?
There is no single answer; it depends on the cause, the dose and whether oral or intravenous iron is used. Repeating too early gives a misleadingly high value. Your doctor will set the interval, often after reviewing haemoglobin response first.
Որքա՞ն ժամանակ է տևում ամբողջ գործընթացը։
The blood draw itself takes a minute or two, and most people are in and out of the collection area within about ten minutes. Laboratory analysis and report release times vary between units, so ask your collection centre when the report will be available.
Is ferritin included in a standard anaemia profile?
Not always. Packages sold as "anaemia profile" or "iron test" vary between laboratories; some include only CBC, serum iron and TIBC. Check the itemised list before booking, and ask your doctor exactly which tests they want performed.
Why is my ferritin low when my haemoglobin is normal?
Iron stores are consumed before haemoglobin falls. This is called iron deficiency without anaemia and can still cause fatigue, hair fall and restless legs. It is worth investigating the reason for the loss rather than only correcting the number.
Can diet alone correct a low ferritin?
Diet helps maintain iron stores but is often too slow to correct an established deficiency, particularly with ongoing blood loss. Vegetarian sources are less readily absorbed than haem iron. Treatment decisions, including whether supplements are needed, should come from your doctor.
Do I need a ferritin test before donating blood?
Blood banks screen haemoglobin, not usually ferritin. Regular donors can nonetheless develop low iron stores over time despite an acceptable haemoglobin. If you donate frequently and feel persistently tired, discuss ferritin testing with your doctor.
