Endometriosis is a chronic gynecologic disease that affects women of reproductive age. It is characterized by the development of endometrial tissue outside the uterine cavity. The main clinical manifestations of the disease are chronic pelvic pain, severe pain during menstrual cycles, pain during sexual intercourse, constipation , and impaired fertility.
Ovaries and the pelvic peritoneum are the most common sites for developing endometriotic lesions. Lesions can also develop in other areas, including fallopian tubes, the abdominal wall, bowel, cervix, bladder, and vagina. Diagnosis involves clinical examination, gynecologic examination, pelvis and laparoscopy, ultrasound scanning, and in some cases, MRI.
Laparoscopy plays a major role in the management of endometriosis. It is a minimally invasive or "keyhole" surgery, allowing for smaller incisions, which leads to faster recovery and shorter hospital stays for patients. Additionally, laparoscopy offers cosmetic advantages by leaving smaller scars and reducing the risk of infection compared to traditional open surgery.
In mild and moderate endometriosis, laparoscopy not only helps in diagnosing the condition but also treats the disease simultaneously. After successful surgery, most patients become symptom-free, and a significant number of women struggling with infertility are able to conceive naturally or with mild assistance (such as ovulation induction agents).
However, recurrent endometriosis is very common, with a recurrence risk of 20 % to 30% within 5 years of surgery. If left untreated, endometriosis can lead to more severe complications, such as kidney damage. In severe cases, patients may require pelvic clearance, which involves the removal of the uterus, ovaries, and affected tissue.
By combining its diagnostic and therapeutic roles with minimally invasive techniques, laparoscopy remains an effective and efficient option for treating endometriosis, providing improved patient outcomes and faster recovery.
